Job description

Central City Concern (CCC) is an innovative nonprofit agency providing comprehensive services to single adults and families impacted by homelessness, poverty, and addictions in the Portland metro area. We hire skilled and passionate people to meet our mission to end homelessness through innovative outcome-based strategies that support personal and community transformation.
The Blackburn Center is an innovative program that will offer integrated housing, health care, and employment services with the long term goal of eliminating homelessness. We seek to change how these services are delivered to all people, especially vulnerable populations.
RESPONSIBILITIES:

  • Conduct screening and assessment of potential clients to determine treatment needs, placement, and diagnosis; develop an individualized treatment plan to reflect these
  • Participate in direct treatment of clients by:
  • Attending clinical staff meeting and case conferences in order to facilitate individual treatment plans for clients;

    • Providing group counseling to clients as needed.

  • Provide case management to clients on an as-needed basis
  • Conduct treatment groups and provide individual counseling as needed.
  • Adhere to agency norms for completion of all clinical documentation, including but not limited to assessment, treatment plan, progress notes, monthly reports to referral sources, and service conclusion summaries.
  • Participate in all Quality Assurance activities.
  • Participate in clinical meetings.
  • May facilitate outpatient treatment groups for driving under the influence (DUI) or domestic violence interventions (DVI).
  • Work as a member of the Blackburn Center multi-disciplinary team to deliver comprehensive, client centered and trauma informed integrated services.
  • Actively participate in the integration of services at the Blackburn Center and identify new opportunities to improve collaboration between and within service and care teams while carefully following information-sharing guidelines.
  • Proactively identify opportunities for improvement within Blackburn Center workflows and logistics. Participate in review of Blackburn Key Performance Indicators; review and contribute to continuous improvement opportunities and plans.
  • Build understanding of and appreciation for diversity among Blackburn Center clients and support their connection to culturally responsive and culturally specific services as requested.
  • Attend all mandatory CCC trainings in a timely manner.
  • Document client services in electronic health records within 72 hours.
  • Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
  • Perform other duties as assigned.

COUNSELOR II ADDITIONAL DUTIES
  • Supervise practicum students when needed.

  • Ability to work courteously and effectively communicate with the general public, clients, medical personnel, corrections personnel, police, and co-workers.
  • Ability to provide quality documentation in an electronic health record within 72 hours of service provided.

QUALIFICATIONS:
  • Must have High School Diploma or GED, advanced degree preferred.
  • The following requirements for CADC:

    • SUDS Counselor R requires CADC R
    • SUDS Counselor I requires CADC I
    • SUDS Counselor II requires CADCII

  • Bilingual A&D Counselors must demonstrate proficiency in secondary language.
  • Must meet CCC privileging requirements as required by FTCA.
  • Must have CPR training and certification prior to start.
  • Must have experience working in chemical dependency treatment, preferably with outpatient adults who have been adjudicated to treatment and/or people with co-occurring mental health disorders.
  • Must be able to demonstrate a working knowledge and skills in group facilitation.
  • Must be able to demonstrate working knowledge and skills in evidence-based treatment methods utilized in the treatment of addiction and trauma-informed care.
  • Knowledge and skills in de-escalation methods or ability to be trained in de-escalation methods.
  • Knowledge and skills in crisis intervention.
  • Must be able to demonstrate knowledge of community and social service agencies with the ability to provide case management services as needed.
  • Must be able to work a range of hours.
  • Preferred understanding of specialized DUII/DVIP system requirements as set forth by the State and referral sources.
  • Preferred experience providing DUII/DCJ compliant treatment services.

If delivering Domestic Violence Intervention services must have:
  • Completion of 40 hours of batterer's intervention training or able to do so within six months of hire date.
  • Completion of 40 hours of domestic violence advocate training or able to do so within six months of hire date.
  • Meet Department of Justice requirements for Domestic Violence Group Facilitator which includes fingerprints and specific hours of education and training. Includes continuing education of 32 hours bi-annually.
  • The OAR prohibits A&D program staff from having within the previous 3 years a DUI conviction or diversion, a felony conviction, or a conviction under the Uniform Controlled Substances Act.
  • Must adhere to agency's non-discrimination policies.
  • Ability to effectively interact with co-workers and clients with diverse ethnic backgrounds, religious views, cultural backgrounds, life-styles, and sexual orientations and treat each individual with respect and dignity.
