Substitute Teacher Information Packet
PLEASE READ THE INSTRUCTIONS BEFORE FILLING OUT THE APPLICATION

1. Fill out Substitute Teacher Application online.
2. Upload a copy of HS Diploma or GED with the application (or submit a copy to the Business Office)
3. Go to the Business Office (355 S Osage Ave) to turn in Background Info and to pick up forms
4. OSBI/Fingerprint Information (see below for details)
5. Direct Deposit Authorization form
6. Voided check or an official document from your financial institution
7. W-4 forms
8. I-9 form
9. Worker's Compensation Notice
10. Loyalty Oath
11. OK New Hire Reporting Form
12. Assurance Statement
13. Watch a 35 minute Substitute Orientation video

*4-12 forms must be picked up at the Education Service Center
OSBI and Fingerprinting Information:
The State Department of Education requires a fingerprint background check for all Skiatook Public Schools' certified and non-certified employees.
If your background check has been processed by another Oklahoma school district within the last five years and you can obtain a letter of good standing from that district along with the background check results, it may be accepted upon review.
To complete your Background check and Fingerprinting:

• Use one of the OSDE sites to do both the fingerprinting and the background check. Payment is made during the appointment or upon scheduling.

1. Visit identogo.com or call 1-877-219-0197 to schedule your appointment.
2. If using the website to schedule, select Digital Fingerprinting then select "Oklahoma" for the state.
3. Choose a convenient location
4. Use Service Code: 2B7KRR
5. YOU MUST BRING YOUR RECEIPT IN. We will need it to access the background check result.

What happens after your application and forms are received?

1. You will be allowed to work as a Substitute Teacher unless your background check results come back negative.
2. You will receive an email from Frontline (where to access available substitute postions)
3. You will receive an email from WenGage (where to access pay stubs). If you have a student in our district, you may receive an email from Skiatook Public Schools.
