Job Summary

The Health and Wellness Coordinator is responsible for completing assigned nursing related services to each client and for linking clients to the physicians and to outside medical resources. They inform the clinical team about medical issues and is available for emergency medical situations. The Health and Wellness Coordinator provides all services under supervision of the Executive Director or Medical Director. The Health and Wellness Coordinator is responsible to assist with medical and history screens, providing routine nursing care, and assisting the Executive Director in overseeing the medical and psychiatric management of the client in coordination with the attending physician.

Responsibilities and Duties

Assist in completing the following documentation within 24 hours of the client’s admission to the program: Admission Note, Nursing Assessment, Pre-Screen Testing, HRS Consent Form, Patient Health Examination

Transcribes Physician’s Orders and Implements all Medical Protocols as ordered.

Confers with and assists the Medical Doctor and Psychiatrist.

Provides documentation which reflects services for each client in accordance with licensure standards.

Meets with clients requesting to see the doctor to determine the urgency of their medical needs and expedite treatment as required.

Effectively communicates concerns about client health to the Executive Director and follows up as necessary.

Under the supervision of the Executive Director & Director of Nursing, assists in educating clients about health problems and medical care.

Facilitates medication education for new clients and for all other clients starting new medications.

Meets with therapists to provide information about client medical problems.

Assists the treatment team in incorporating medical issues into treatment planning.Takes verbal orders from the Doctor or Medical Director, documents them in the client record, and carries them out accurately and professionally.

Other duties as needed.

Qualifications and Skills

LVN license or similar medical qualifications is a must.

SchedulingNight Shift (12am - 8:30am) full time'

Salary: $33.00 - $35.00 per hour
