Family Guidance Centers, Inc. (FGC), a not-for-profit behavioral healthcare organization, treats and prevents Substance Abuse, as well as an array of other related problems. Since 1969, FGC's comprehensive programming has educated and guided individuals toward their full potential as productive members within their own families and our society.
FGC’s adult and adolescent programming provides a flexible continuum of care to best address the individual needs of each participant. Through integrated and comprehensive services, including continuous assessment, participants are able to access levels of care and specific services to create an individualized treatment approach. Further, FGC provides special and high-risk populations priority access to specialized programming.

Essential Duties and Responsibilities:
- Complete SAMMS intake assessments for new patients
- Complete an individualized treatment plan for each patient on caseload, addressing all needs identified in the psychosocial assessment
- Provide group and individual sessions to patients in accordance with their treatment plan problems, goals, and objectives
- Properly document all services provided in individualized progress notes per SUPR, Joint Commission, and FGC regulations, standards, and policies, summarizing the patient’s status and ongoing treatment needs
- Review and evaluate patients’ progress and continued stay in treatment in relation to measurable goals and objectives described in treatment and care plans. Modify treatment plans to comply with changes in patient status
- Plan and assist with the implementation of aftercare and continuing recovery plans once patients are discharged from treatment programs
- Work with patient and other staff, when necessary, to deescalate and assist with patient emergencies and crisis situations
- Coordinates care with other treatment providers, DCFS, probation/parole, and others as necessary, to help ensure patient needs are met and increase positive treatment outcomes
- Participate in any treatment team staffings
- Make appropriate patient referrals to outside agencies for the provision of ancillary services
- Maintain patient confidentiality at all times in accordance with 42 CFR and HIPAA regulations
- Develop a working knowledge of FGC applicable Standard Operating Procedures (SOP)
Schedule: Monday thru Friday 6:00 am - 2:30 pm with Saturday rotation

Pay Rate: $36,000 - $46,000
Experience and Skills:
Education and/or Experience: Associate degree (or higher) in Addiction Counseling, Psychology, Social Work, Counseling or related field is preferred; Experience in/with substance use, detox programs, medication-assisted treatment, behavioral healthcare and/or community relations encouraged to apply.
Certificates, Licenses, Registrations: IAODAPCA Certified Alcohol and Other Drugs Counselor (CADC) required, or able to obtain within allotted time of hire date.
