Job Summary:

The Stroke Coordinator is fundamental to the development, implementation, and evaluation of the Stroke Program with a focus on the provision of quality services for patients who are experiencing stroke from a multi-disciplinary perspective throughout the continuum of care. This individual works collaboratively with the Stroke Program Medical Director in a mutually supportive relationship, which is essential to the success of the program.

The Stroke Coordinator is an expert nurse who provides leadership for the organization in services and systems necessary for a collaborative approach to stroke care. This position plans, implements, and evaluates clinical programs to achieve high quality, patient-focused outcomes that advance patient and family care and nursing practice in stroke. The Stroke Coordinator is responsible for the development, quality assessment, performance improvement, and inter/intra disciplinary communications within the program and will work collaboratively with all hospital and clinic personnel on shared system and performance improvement projects.


Job Responsibilities include, but are not limited to:

  1. The Stroke Coordinator is responsible and accountable to prioritizing person-centered care within the program's activities with a goal of co-creating with patient and families, wherever possible
  2. Accountable for establishing operational and strategic goals and objectives consistent with the standards set forth by the American Heart Association (AHA) and American Stroke Association, the Joint Commission, and the New York State Department of Health (NYSDOH) to maintain Joint Commission Primary Stroke Center Certification.
  3. Collaborate with the Quality Outcomes, Project Management, Risk Management, Case Management, Clinical Pharmacy, Regulatory Compliance, Nursing Education and Research, and other clinical leaders to lead, develop, and implement quality improvement projects that support quality initiatives specifically related to the Stroke Program.
  4. Coordinates the development, implementation, review, and revision of Stroke Program standards, policies, procedures, and protocols.
  5. Participates in Stroke Alert activations when available to ensure protocols are followed and provide just-in-time guidance as needed.
  6. Functions as a liaison with Emergency Medical Services (EMS), providing feedback and guidance in pre-hospital Stroke processes.
  7. Assists in the coordination of the Telestroke Program.
  8. Apply knowledge in quality improvement methodology to identify, lead, and consult with others for improvement projects and ongoing initiatives. Use a variety of tools and methodologies appropriately to achieve results in improving care and services, including PDSA and DMAIC.
  9. Facilitates the delivery of acute stroke continuing education programs.
  10. Provides stroke-related education to internal and external audiences including planning and participating in educational programs targeting hospital personnel, EMS providers, and Local and Regional communities.
  11. Maintains awareness of current research related to stoke patients and stroke systems of care and as appropriate applies current research to practice within the organization.
  12. Actively participates in internal, local, regional, and national advisory groups to work towards the advancement of Stroke Care.
  13. The Stroke Coordinator will chair relevant Stroke Program committees related to Quality Improvement and regulatory compliance.
  14. Accountable for major data registry abstraction and entry to include Get with the Guidelines through AHA.
  15. Use verbal and written communication skills to establish and maintain effective relationships, facilitate multi-disciplinary teams, and produce written and/or verbal reports. Use knowledge of team process and collaboration to increase effectiveness of workgroups and committees. Develop strong team relationships with staff members in similar roles.
  16. Present and participate in applicable and relevant Quality Committees/Morbidity and Mortality Conferences to embed and drive quality improvement.

Qualifications

Required:

  • Education: BSN Required, master's preferred
  • Licensure: Current unrestricted NYS RN License
  • Work Experience: 3 to 5 years of clinical experience (inpatient and outpatient)
  • Specific Skills: Excellent organizational skills, attention to detail, excellent communication skills (verbal and written). Experience working with Quality, Data, Performance Improvement, Customer Relations programs highly desirable.
  • Physical: Ability to stand, sit or ambulate for long periods. Ability to mobilize freely around units, transport self to site locations, and perform required data entry into EMR and alternate data management systems. Ability to transport self to customer base.
