SalaryResponsibilities:
- Provide patient-centered pharmaceutical care and medication therapy management
- Ensure accurate and timely dispensing of medications
- Collaborate with healthcare professionals to optimize patient outcomes
- Conduct medication reviews and provide drug information to patients and healthcare providers
- Monitor patient medication therapy for safety and efficacy
- Participate in medication reconciliation processes
- Assist in the development and implementation of pharmacy policies and procedures
- Maintain accurate records of medications dispensed and patient profiles
Experience:
- Strong knowledge of patient service and patient care principles
- Experience in chemotherapy & IV infusion preferred
