The Senior Manager, VBC Care Management in partnership with a Senior Manager, VBC Operations delivers both clinical and operational leadership and is responsible for implementation, integration, coordination, oversight, and evaluation of the overall model of care delivery. The Senior Manager, VBC Care Management ensures evidence-based standards, appropriate scope of practice, consistent and timely care, high performance metrics, and a focus on outcomes that meet or exceed the organization's clinical and financial goals. This position will partner with Operational Leadership and other stakeholders to ensure a seamless system where Panoramic Health achieves maximum quality outcomes, efficient resource utilization, and patient satisfaction.
Responsibilities include:

  • Ensures care model operationalization and provides oversight of daily program activities, including the direct development, mentorship, coaching, and supervision of the RN/CC Dyad team's clinical activities.
  • Oversees and supports the collaboration between interdisciplinary health care professionals, including social work, dietitians, pharmacy, community health workers, and other care team members.
  • Continuously evaluates the overall care workflow and delivery model to assess for efficiency, integration, impact, quality, and safety. Escalates critical issues to ensure timely resolution.
  • Oversees Care Management of the targeted population in an appropriate, efficient, and cost-effective manner. Ensures team has access to all applicable reports.
  • Demonstrates ability to understand, analyze, and utilize population health data to drive informed decisions and assist in clinical prioritization of Care Management tasks.
  • Responsible for leading and organizing clinical reviews of Care Management cases with local Care Management team.
  • Leads and manages the implementation of clinical workflows, standard processes, transitional Care Management, longitudinal Care Management, risk management, care planning, etc.
  • Collaborates with system-wide operations leaders, clinicians, physicians, and administrators to develop, implement, monitor, and achieve set goals in all dimensions of care.
  • Facilitates the local quality assurance program and provides timely feedback, mentoring, coaching, and remediation, as appropriate in partnership with the Sr. Manager, VBC Operations dyad.
  • Participates in compliance initiatives on a system-wide basis and facilitates implementation of such initiatives related to local Care Management. Stays abreast of clinical program innovations through local, regional, and national networking and acts as a subject matter expert within the system.
  • Collaborates with the Medical Management Office in developing and implementing clinical workflows, dashboards, care paths, documentation guidelines, processes, tools, and policies to ensure consistent standards of care and drive variation from the delivery model.
  • Promotes and maintains positive relationships with medical team, VBC colleagues, office personnel, administration, hospital clinicians, post-acute providers, contracted providers, and other communities relative to Care Management.
  • Develops and organizes regional team meetings to develop, implement, and evaluate systems to define, measure, and improve quality and the optimization of the Care Management delivery.
  • Collaborates with VBC Operations Senior Manager on daily operations of Care Management team, including hiring, annual performance appraisals, conflict resolution, and ongoing training and education of team. Builds and supports diverse teams across multiple locations.
  • Demonstrates influential leadership and ability to deploy creative solutions to unique problems.
  • Enforces adherence to the state board of nursing and state nurse practice requirement for respective states.
  • Supports the optimization of home dialysis modalities.
  • Creates best practices for PD education with staff and patients.
  • Actively participates on Panoramic-wide Care Management related teams.
  • Perform other duties and responsibilities as required, assigned, or requested.

Qualifications:
  • Licensed to practice Registered Nursing in applicable state of practice and/or possess Compact Licensure.
  • Bachelor's Degree in Nursing from an accredited institution highly preferred.
  • Five years of leadership experience managing a variety of patient care services in a multi-site healthcare environment is preferred.
  • Experience in a leadership role requiring strong relationships and influence in a matrix environment, requiring work in a fast-paced and changing environment while handling multiple deadlines and priorities.
  • Experience in managing other clinical areas and teams, working closely with physicians, developing clinical strategies, implementing operational efforts, and measuring outcomes.
  • Experience in a role requiring analytical skills, expertise in data management, and using spreadsheets, databases, multiple electronic medical records, internet, email, and scheduling applications.
  • Experience in population health, value-based care models and Medicare programs preferred.
  • Proficiency with navigation of multiple EMRs, web -based applications, Microsoft Outlook, Teams, Excel, Word, and PowerPoint.
  • Must be able to work independently and manage clinical staff in a remote environment.
  • Ruthless attention to detail with excellent follow-up.
  • Proven ability to build and lead diverse and distributed teams from various internal and external organizations across multiple locations.
  • Adept at helping internal and external resources harness and develop strengths in pursuit of successful projects.
  • Consults and provides advice, facilitates discussion, and resolves conflict; establishes trust; builds and uses cross-functional relationships to accomplish work objectives.
  • Ability to promote innovative ideas and accept the risks that are required to lead change.
  • Ability to achieve broadly communicated objectives with a minimal amount of supervision.

  • Ability to make decisions independently and to assume higher-level leadership responsibilities in critical situations.
