Our Home Health Speech Language Pathologists will:
- Assess, plan, develop and provides the patient or responsible caregiver a home health plan of care for Speech Language pathology.
- Identifies patient needs for other disciplines and services.
- Coordinates and evaluates the plan of care with all other disciplines and physicians for an assigned caseload.
- Instructs the patient or responsible caregiver on the projected care plan and expected outcomes as directed by the physician's treatment plan.
- Detailed documentation of visits in EMR system.
Minimum Qualifications:
- Licensed and current registration to practice as a Speech Language Pathologist in New York State required.
- Minimum of one year post-licensure experience in rehabilitation with adult speech/language impairment populations.
- Good computer skills - Electronic Medical Record experience required.
