Full-time | 64 hours every two weeks
JOB SUMMARY:
To provide professional, social work services for Willmar Long Term Care Center patients/families in assigned inpatient and outpatient units/departments. The individual in this position practices social work focusing on patient's/family's psychosocial needs through assessment, discharge planning, patient/family education regarding available resources, advocacy, support, and linkage to community referrals. Represents Rice Memorial Hospital on community boards, task forces and other activities assigned by the director. Is responsible for program development, evaluation, and improvement in assigned clinical area.
EDUCATION:
- Minimum of bachelor's degree in Social Work with 2 years of experience preferred.
LICENSE:
- Licensed by the state of Minnesota.
EXPERIENCE:
- Previous experience in a related setting required.
ADDITIONAL QUALIFICATIONS:
- Social Services degree with agreement to obtain license within 1-2 years.
- Knowledge of the psychosocial aspects of illness/disability/ hospitalization.
- Ability to assess needs of patients and families. Knowledge of community resources and how to access them.
- Requires knowledge and ability to utilize skills in supportive counseling and support group facilitation.
- Knowledge of crisis intervention, vulnerable adult, and abuse regulations.
- Requires excellent verbal and written communication skills.
