Summary Safe Haven Liaison performs social work duties specifically directed toward Veterans enrolled in the Safe Haven program. The Safe Haven Liaison is responsible for the fiscal and contractual oversight and provides clinical, psychosocial and case management services for eligible Veterans and their family members. They will work closely with the VA medical center's other homeless programs, to ensure that coordination is facilitated and Veterans' direct care, referral, and follow up is contiguous. Responsibilities The Safe Haven Liaison manages the daily operation of the CR programs as outlined in VHA Handbook 1162.09 Health Care for Homeless Veterans (HCHV) Program and provides direct patient care services for Veterans in the CR programs. The incumbent develops policies and procedures for program operation and prepares reports and statistics for facility, VISN, and national use. The incumbent manages and controls the use of data related to these programs regarding day-to-day functioning and program evaluation. The incumbent is responsible for managing the programs' budget and fund control points. In conjunction with the Homeless Programs Supervisor, the Contract Residential Coordinator plans, organizes, and directs administrative, operational and patient activities for the CR Programs. The Safe Haven Liaison provides oversight of the CR-funded programs. The liaison performs announced and unannounced site visits of CR programs to ensure that Veterans reside in safe environments, in compliance with HCHV guidelines. They coordinate and participate in the site inspections for each program and reviews all inspection packets for completeness and accuracy and forwards to the appropriate offices. Intervenes as necessary to facilitate compliance or corrective actions and, when appropriate, develop, facilitate, and monitor the corrective action plan. The Safe Haven Liaison is responsible for the monthly billing and monitoring of bed utilization. They work closely with Medical Center/VISN fiscal department and the appropriate CR Contracting Offices. The liaison conducts psychosocial assessments to determine the needs of Veterans and family members. Demonstrates an ability to work with a wide variety of individuals from various socioeconomic, cultural, ethnic, educational and other diversified backgrounds. Possesses basic knowledge of human development and behavior (physical and psychological) and the differential influences of the environment, society and culture. Formulates and implements a treatment plan. Identifies the Veteran's problems, strengths, weaknesses, coping skills, and the assistance needed. Provides counseling and/or psychotherapy services to individuals, groups and families. Social Workers must practice within the scope of their license or certification. The liaison conducts a suicide risk assessment and all requirements per the Suicide Prevention guidelines. Verifies the Veteran's status, eligibility for VA medical care, and eligibility for program participation. Assists Veterans in obtaining permanent housing. Possesses basic knowledge of psychosocial treatment modalities. Implements treatment modalities in working with individuals, families, and groups to achieve treatment goals. This requires judgment and skill in utilizing supportive, problem solving, or crisis intervention techniques. The liaison serves as an advocate on behalf of Veterans to ensure comprehensive service delivery. Links and refers Veterans to the VA Medical Center, VA Regional Offices and/or community agencies. Conducts regular case discussions and treatment planning with the Veteran, CR-funded program staff and the CR Liaison. The liaison also provides supportive case management services to assigned Veterans and significant others, consistent with the goals and methods of the Healthcare for Homeless Veterans (HCHV) program and with all relevant Medical Center policies and procedures. Supportive case management services are characterized by assertive outreach, individualized treatment planning, a multidimensional orientation and the intensive delivery of services. Have a masters degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. Licensure. Persons hired or reassigned to Social Worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social http://vaww.va.gov/OHRM/T38Hybrid/. Failure to Obtain License or Certification. In all cases, Social Workers must actively pursue meeting state prerequisites for licensure or certification starting from the date of their appointment. Failure to become licensed or certified within the prescribed amount of time will result in removal from the GS-0185 Social Worker series and may result in termination of employment. Loss of Licensure or Certification. Once licensed or certified, Social Workers must maintain a full, valid, and unrestricted independent license or certification to remain qualified for employment. Grade Determinations: Social Worker, GS-11 Experience and Licensure. Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non- VA experience) and licensure or certification in a state at the independent practice level. NOTE: For appointment licensure or certification at this level please refer to paragraph 3c. OR Education. In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: (a) Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services. (b) Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. (c) Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i .e acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan. (d) Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals. (e) Ability to provide consultation services to new Social Workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.
