Summary
This position serves as a front line Social Worker and liaison for the VA, supporting the Office of Integrated Veteran Care (OIVC) Program. Position works primarily with non-VA community hospital case managers across the Tampa VA catchment area to assist in discharge planning, safe transfers and smooth placements for Veterans to the next medically appropriate and ordered level of care, i.e. VA Funded LTACH and/or VA Contract Nursing Home (CNH, CCN).

Duties
Duties include, but are not limited to:
  • Care provided may include assessments and case management of:
    • Placement of Veterans from non-VA medical facilities to the next level of care.
    • Responsible for assisting Office of Integrated Veteran Care (OIVC) patients with VA contract referral requests.
    • May be responsible for assisting with other patient placements as determined by assigned supervisor.
  • Independently performs case finding functions and outreach, as well as identify and screen patients for psychosocial needs. Implements treatment modalities, provide counseling or psychotherapy for individuals, families and groups.
  • Conducts psychosocial assessments, develop treatment plans in collaboration with the Veteran/family and with the interdisciplinary treatment team, and offers psychosocial interventions and measures outcomes of interventions.
  • Coordinates community-based services, including information and referral for those services including VA programs, other government programs and community agency programs.
  • Makes rapid assessments and developing crisis management plans for admission to acute, short term, and/or long term placements. Utilizes multiple resource directories in addition to previously developed resources to meet the demands of a crisis.
  • Independently identifies high-risk patients and provides case management services. Has working knowledge and experience in use of medical and mental health diagnoses, disabilities and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology.
  • Establishes and maintains effective therapeutic relationships with Veterans/patients and their families and/or caregivers. Works with patients and families who are experiencing a variety of psychiatric, medical, and social problems utilizing individual, group and family counseling and therapy skills.
  • Maintains general knowledge of Veterans' benefits and services relating to special programs, service connected compensation and non-service connected pension. Maintains advanced knowledge of community resources as well as VA programs that support the psychosocial and financial needs of Veterans living independently in the community and facilitates referrals.
  • Has working knowledge and skill in the use of word-processing, data management and other computer systems, especially those programs in use by VHA.
  • Other duties as assigned.

Work Schedule: Monday - Friday, 7:30 AM - 4:00 PM

Applicants must meet the requirements by the closing date 12/06/2023 of this announcement.
Basic Requirements:
  • United States Citizenship: Must be US Citizen. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
  • Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may NOT be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
  • Licensure: Persons hired or reassigned to Social Worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.

May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations: In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates.
GS-11 Social Worker candidates must have the following:
  • Experience and Licensure: Completion of a minimum of one (1) year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non-VA experience) and licensure or certification in a state at the independent practice level.

OR
  • Education: In addition to meeting basic requirements listed above, a doctoral degree in social work from a school of social work may be substituted for the required one (1) year of professional social work experience in a clinical setting. You must attach transcript to support this qualification.

  1. Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services.
  2. Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds.
  3. Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan.
  4. Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals.
  5. Ability to provide consultation services to new Social Workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.

The full performance level of this position is the GS-11.
Physical Requirements: The physical demands of the work requires use of fingers; moderate walking and standing; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in on eye to 20/20 and to 20/40 in the other; hearing aid permitted; clear speech; emotional stability; moderate duty. Ability to drive/operate government vehicle if needed.
Education
