Responsibilities:
- Assess, care plan and organize social welfare needs for residents residing in a nursing home. Our maximum number of residents is 45 whose average age is 95.
- Develop and implement engaging programs that promote socialization, physical activity, and mental stimulation
- Coordinate with staff and volunteers to ensure care plan goals are carried out effectively.
- Assess residents' needs and interests.
- Assist residents with activities of daily living (ADLs) as needed
- Maintain accurate resident clinical records.
- Communicate regularly with residents, families, and staff to ensure satisfaction and address any concerns
Requirements:
Applicant must be licensed or a degreed Social Worker.
- 4 years previous experience as Social Worker in a long-term care facility.
- Knowledge of senior care, dementia care, and memory care practices
- Strong organizational skills with the ability to manage schedules and coordinate multiple resident simultaneously
- Excellent communication skills, both verbal and written
- Compassionate and patient demeanor when working with elderly individuals
- Ability to work collaboratively with a team of healthcare professionals
- Flexibility to adapt to changing needs and priorities
