As our Mental Health Social Worker you’ll perform social work assessments and interventions for inpatients, outpatients, and emergency department patients. Working collaboratively with all healthcare team members, you will develop and implement treatment plans that support patient-centered care for individual patients and the medical community. Being dedicated to patients and family-centered care, you value the personal self-determination of your patients. To succeed in this role, you must manage multiple priorities simultaneously and engage in creative, compassionate, and ethical problem-solving.
Responsibilities
- Perform comprehensive assessments and develop treatment plans that integrate the medical, social, and resource issues that impact individual patients, families, and at-risk populations
- Establish and maintain professional relationships with physicians, case managers, nursing staff, county agencies, community resources, patients, and families.
- Serve on hospital committees and develop and present in-services to Hospital staff.
