Job Duties

This position supports the facility’s mission by facilitating the admission and discharge process for individuals, serving as agency liaison to families, community agencies, and state or private institutions, while advocating on behalf of the individual in regard to benefits and rights, and counseling individuals or family members as needed.
Ability to communicate effectively and maintain productive working relationships, produce reports, perform Needs Assessments for discharge planning and work collaboratively with peers, supervisors, individuals, family members and community partners. Ability to work independently, while maintaining quality performance and organization of multiple tasks in a busy, time sensitive office environment. Ability to interpret and apply written and verbal policies, procedures and guidelines. Knowledge of case management services. Strong organization skills. Ability to maintain appropriate filing systems. Ability to utilize time management skills in order to meet deadlines and successfully prioritize while carrying out directives from supervisor.
Knowledge of Olmstead and philosophy of most integrated setting, available community resources, and applicable federal, state and departmental regulations and policies; including familiarity with Medicaid, Medicare, Social Security, SSI and HIPAA. Ability to maintain strict confidentiality when dealing with sensitive and confidential information.
This position is considered essential personnel and would be required to work in the event of an emergency or facility need. All SEVTC positions are required to work different shifts and/or overtime as requested.
This position qualifies for limited telework as approved to meet the overall needs of the facility.

Minimum Qualifications

Experience as a Social Worker, especially experience working with individuals who have an Intellectual Disability.

Additional Considerations

Intermediate skill level in the use of Microsoft Office (Word, Excel, PowerPoint, Access) and Google G Suite. Demonstrated skill and proficiency with data entry and database management.

These positions are hourly/wage and not eligible for benefits. Hourly (wage) employees may not work more than 29 hours a week and are limited to working 1500 hours in a year.
