Form of workDESCRIPTIONUnder the general description of the Human Services Supervisor - Community Based Services, this position supports the mission of Blue Earth County Human Services by effectively and efficiently delivering essential services to families and individuals so they can achieve their potential through active community participation. The Social Worker - Children's Mental Health Manager will be responsible for providing Children's Mental Health Targeted Case Management (TCM) assisting children diagnosed with a severe emotional disturbance (SED) and their families with assessing, accessing, coordinating, monitoring, and planning Mental Health services. This position provides Children's Mental Health Case management, performs assessment services, maximizes case revenue, performs crisis intervention, completes written documentation, and participates in professional staff development.DUTIES / RESPONSIBILITIESESSENTIAL FUNCTIONS:* Provides Children's Mental Health (CMH) Target Case Management (TCM) Services in accordance with Minnesota statutory requirements and Department of Human Services guidelines.
Performs intake duties as assigned, including but not limited to completion of intake referral information, appropriate consents, and intake records, gathering of required documents to determining eligibility for case management services and provides back up intake as needed for assigned units. Meets with children and families to assess strengths, service needs, and available community supports. Develops person-centered case plan and goals.
Coordinates and monitors the provision of services between families and service providers to assure services are being provided, utilized, and effectively meets the needs of the child and family. Meets with the child and family at least monthly to review case management including assessment, planning, referral/linkage to community-based services and monitoring/service coordination. Opens, maintains, and closes case files.
Case management services may include helping the child/family obtain needed Mental Health services, social services, educational services, health services, vocational services, recreational services, and related services in areas such as volunteer services, advocacy, transportation, and legal services.* Performs Functional Assessment services. Assesses need for type of interventions and care required. Presents placement consideration requests to Juvenile Screening Team.
Completes referral process for out of home placements as needed. Contacts placement providers and completes placement services documentation. Develops the Out of Home Placement Plan with the family, child, and service providers.
Monitors placement through monthly face to face contact and telephone interactions. Assesses progress towards goals and actively participates in discharge planning.* Consults with Clinical Supervisor, Community Based Services Supervisor, Team Members, Service Providers and Client Care Providers. Discusses and reviews Individual Family Community Support Plans goals and objectives with Clinical Supervisor, client, family members, and other treatment team providers.
Determines if goals and service interventions are appropriate. Reviews with Clinical Supervisor the progress of client toward determined goals. Consults with Clinical Supervisor and Community Based Services Supervisor regarding clients experiencing a crisis and assists in determining appropriate support services.
Coordinates revision of community support services as needed.* Performs crisis intervention. Responds to crisis of individuals from assigned caseload. Provides support and assistance to peers with cases on an as-needed basis.* Applies all applicable laws, regulations, and agency expectation regarding Children's Mental Health Targeted Case Management Services.
Prepares, disseminates, and maintains required documentation.* Performs On-Call duties as assigned.* Promotes a diverse, culturally competent, and respectful workplace.* Interacts positively with internal and external customers. Communicates and interacts effectively and respectfully with employees, supervisors, individuals from other organizations, and customers.* Performs the physical and mental demands of the position.* Attends work during regularly scheduled hours.ADDITIONAL FUNCTIONS:* Participates in Professional Staff Development. Associates with agents and departments from other counties and states to learn about and maintain professionalism.* Performs Other Related Functions as Apparent or Assigned.QUALIFICATIONSMINIMUM QUALIFICATIONS:* Bachelor's degree in social work, sociology, psychology, or related field* 1-year (2,000 hours) experience of supervision provided by a Mental Health Professional working in Children's Mental Health services.PREFERRED QUALIFICATIONS:* 2-years experience providing Children's Mental Health Case management for a county, state, or contracted provider.CONDITIONS OF EMPLOYMENT:* Must possess a valid driver's license and be able to drive in the State of Minnesota.* Must successfully pass a background check.* Must comply with federal and state laws and organizational and departmental policies and regulations.* Must comply with data practices policies and standards relative to not public data. Any access to not public data should be strictly limited to accessing the data that are necessary to fulfill the employment responsibility. While data are being accessed, incumbent should take reasonable measures to ensure the not public data are not accessed by individuals without a work reason.
Once the work reason to access the data is reasonably finished the not public data must be properly stored according to county policy and the Minnesota Statutes.HOURS OF WORKMonday through Friday from 8:00 a.m. to 5:00 p.m. This position allows for flexible hours and may require some evening, weekend, and overtime hours.
