Social Worker - Adhc (Mandarin English) - La/ Oc County

Work with a dynamic multidisciplinary team of health care professionals!

Pay starting from $26-$28/hr up to $30 (DOE) and may be higher for well experienced MSW

Provide social services in adult day health care/ community-based adult services center

- Conduct psychosocial assessments of participants

- Develop and implement each participant's individual plan of care (IPC)

- Collaborate with participant, family & multidisciplinary team to achieve goals

- Provide information, referral, and counseling as needed

- Assess home safety, living conditions and family relationships

- Document all services in participant progress notes and files

* Bachelor's degree in social work or closely related fields and experience in healthcare required; MSW preferred (no experience required for MSW)
