Social Worker

Clover Rest Home is a 33-bed capacity sub-acute and long-term care facility is seeking for a Social Worker to provide services to meet the social and/or emotional needs of the residents.

• Collaborate with the interdisciplinary team to promote and protect resident rights and the well-being of all patients/residents. This includes educating staff on the role of the social services.
• Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social services needs.
• Hold meetings with patients/residents, families, and significant others as requested and to provide support and information so as to improve quality of life.
• Participate in Utilization Review meetings to assure best quality of care for the residents.
• Complete Admission paperwork and interview of new and existing residents.
• Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers.
• Establish relationships and maintain contact and referral flow with community based agencies/services.
• Complete MDS criteria.
• Assist with Marketing for new admissions and connections with hospitals.

Requirements:

• Master's
• LCSW
• Must be licensed in New Jersey.
• SNF/LTC experience and knowledgeable with federal and state regulation.
