Job Description
Title: Social Worker
Location: Basking RIdge, NJ
Schedule: Monday - Friday 8AM-5PM
Note: This position is a contract on W2, and is NOT open to C2C
Duties:
- Performs comprehensive psychosocial assessments: Identifies behaviors and issues that affect coping at the end of life Identifies issues that may have an impact on the safety of patients and families Identifies availability of support systems to meet patient/family goals Identifies bereavement risks and communicates these appropriately
- Collaborates with other disciplines in the development of the plan of care: Provides input to the development of short-term and long-term goals Participates as an active member of the interdisciplinary team Collaborates with external resources in developing the plan of care.
- Collaborates with other disciplines in the implementation of the plan of care: Counsels patients and families in strategies to cope with issues of loss and terminal illness Participates with other disciplines in family conferences Provides referrals to appropriate community services Facilitates movement between levels of hospice care.
- Evaluates the effectiveness of social work services: Modifies interventions based on input from the IDT Serves as a resource to team members.
- Maintains high standards of professional practice: Completes documentation according to agency guidelines Maintains current knowledge of community resources and eligibility requirements Maintains knowledge of hospice Medicare benefit and other insurance coverage.
- Participates in the agency s Performance Improvement program: Participates in utilization review activities.
- Provides appropriate bereavement services for survivors: Accurately identifies bereavement risks Develops and implements bereavement plan of care based on input of IDT and assessment of risk Plans, implements and supervises bereavement groups Participates in community outreach and education programs designed to assess and manage bereavement needs and services
- Assists in the development of agency policies and procedures: Provides orientation to new employees. Participates in developing/revising Job Functions.
- Assists with growing agency business: Participates in community education and outreach Assists with orientation and training of hospice volunteers Cross trains to learn new skills Accepts other assignments as requested to support agency.
Skills: At least one year post masters social work experience Current NJ SW license
Education: Masters degree in social work - Required
