Job description

St. Luke’s Health System in Twin Falls is seeking a Care Coordinator to join our team.

 

Care Coordination is a process that facilitates connecting children and their families with appropriate services and resources in a coordinated effort to achieve good health. The Care Coordinator is part of the pediatric medical care team and works side by side with physician and nurses in the hospital and clinic setting. The Care Coordinator is a direct point of contact for the patient and family in navigating medical services through St. Luke’s Children’s Hospital, as well as community services and school entities.

Our pediatric team has more than 150 skilled pediatricians and pediatric specialists, including a full-service hospitalist program who work with providers from around the region to provide high quality care to more than 100,000 pediatric patients annually. Additionally, we have a Level IV NICU and a 13-bed PICU, the first in the state of Idaho. St. Luke's Children's Hospital is also developing a pediatric trauma program. 


  • Works as consultant, educator, or community liaison to facilitate collaborative coordination of patient care.
  • Identify and document patient and family needs related to treatment goals.
  • Evaluates patient progress toward outcomes by monitoring the goals, care plan and revising plan of care in consultation with clinical care team as necessary.
  • Assists the care team by identifying potential barriers that impedes access to care. Implements plan to address barriers.
  • Responds to patient and family needs and acts as an advocate.
  • Coordinates transitions of care and provides educational materials and resource management.
  • Coordination of care with primary care and/or sub-specialty providers.
  • Perform other duties and responsibilities as assigned.

  • Education: Bachelors’ Degree in Social Work or related field.
  • Experience: 2 years’s of related experience.
  • Licenses/Certifications: None
