The Social Work Case Manager utilize professional medical Social Work skills to assess patient and family needs for complex discharge planning, complex placements, information and referral, psychosocial and counseling support needs, and establishes plans for effective management of the identified needs. Assessment, planning, intervening and reassessment are performed in a collaborative environment. This position also includes proactive, individualized planning for patients’ progress across the continuum that optimizes quality of care, patient satisfaction, utilization, and cost.
Required:
A. Registered Nurse or Master’s or Bachelor’s prepared Social Worker from a program accredited by the Council of Social Work Education with a current Maryland license.
B. Case management certification is preferred but not mandatory
C. Three to five years of case management experience preferable but not mandatory.
D. Experience in performing case management, utilization review, discharge planning, or quality/performance review preferred but not mandatory.
E. Demonstrates effective interpersonal and communication skills.
F. Ability to apply creative problem solving skills.
