We are seeking a Case Manager who actively engages with youth, families, and internal and external resources to ensure the needs and goals of the children are met.
Schedule: Monday-Friday, 8 am-5 pm. Occasional travel is required.
Starting Wage: $23.50 BSW, DOE or $25/hr Licensed BSW (LBSW), DOE
What you will do: You will oversee the treatment planning process for 8-10 families by collaborating with a multidisciplinary team to create and maintain individual treatment plans.

- Partner with the family and listen to what they identify as goals.
- Empower families by teaching them how to communicate with each other, create daily schedules, and manage behaviors and medications.

- Sharing the information and goals within the treatment plan with all service areas involved.
- Building excellent rapport with guardians and referrals.

We are persistent and proactive in sharing information with others. We support each other in our varying roles, recognizing that we all have different areas of expertise and strength.
- Translate information gathered during intake into an individualized treatment plan.
Qualifications:
- 21 years of age
- Ability to pass a background check, including criminal and abuse
- Ability to pass a Motor Vehicle Check
- Must have a Bachelor's Degree in Social Work or be graduating this semester with this degree (no other degree can be accepted per ND licensing rules)
