Dakota Home Care is looking to hire a Part-Time Nurse Case Manager. Our Nurse Case Manager position requires an RN degree with at least 3 years of clinical experience. Experience in pediatric care & home health care is a plus, vent and trach experience preferred. The Nurse Case Manager is responsible for admissions, care planning, and overseeing care in the client's home. Dakota Home Care is a licensed home healthcare agency located in Fargo, ND. Hours are Monday through Thursday 8 am to 2 pm and Fridays 8 am to 12 pm. 28 hours a week.
