Job Description
JOB SUMMARY:
- Responsible for the coordination and collaboration of hospital and community based Social Services for patients, families, medical staff, hospital departments, and community agencies to ensure efficient and effective utilization of medical social work.
- Provides services in accordance with the mission, vision, and values of Hancock Health.
- Performs other duties as assigned.
QUALIFICATIONS:
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
- Completes Biopsychosocial Assessments for patients of Hancock Health to identify SDOH needs and provide referrals to community resources to address such needs.
- Completes Psychiatric Risk Assessments for at-risk individuals to determine level of care, provides recommendation for Psychiatric placement in collaboration with medical staff, and completes placement as needed with inpatient acute stabilization.
- Provides Crisis Intervention and utilizes de-escalation skills to address concerns.
- Will arrange transportation services for patients who have been identified as needing transportation support from the Emergency Department including but not limited to Uber and RIDE Hancock.
- Develops and maintains thorough knowledge of referral resources throughout the service area as well as develops and maintains positive working relationships with identified resource groups.
- Aids other inpatient units as required including but not limited to the Andis Maternity Unit and CICU.
- Will provide inpatient units with supportive services and resources including but not limited to Substance Misuse Treatment and Recovery support, assist with OB discharge planning, support for parents of infant loss, and resources regarding adoption including support for birth parents and adoptive parents as identified.
- Provides support within the Emergency Department related to DCS, APS, and SANE.
- Keeps a data tracking system to track items including but not limited to, referrals received, and services provided.
- Provides in-service education and consultation in understanding the social and emotional factors related to health problems for members of the health care team.
- Maintains social service program through ongoing development, implementation and/or revision of policies, procedures, and guidelines for Social Services.
- Participates in Quality Improvement activities as identified.
- Maintains accurate and timely records and documentation within identified EMR according to department policy.
- Represents hospital at meetings and on various committees which address social needs of patients.
- Provides mentorship and oversight of Social Work students.
- Adheres to Hancock Regional Hospital Code of Ethics, thereby exemplifying the hospital’s values.
