POSITION PURPOSE

The Social Service Worker (SSW) is responsible for assessing patient/family psychosocial needs, planning, implementing, procuring services for, and evaluating care needs while meeting with members in their homes. As an SSW your goal is to help improve the quality of life and health literacy. The SSW helps to coordinate, monitor, and ensure information is shared in an appropriate and timely primary, across the continuum of care. The SSW will promote the effective utilization of tools and monitor healthcare resources.

This position requires travel to Las Vegas, around Clark County, and surrounding areas.

DUTIES & RESPONSIBILITIES

  • Provide case management field-based services to the assigned client population.
  • Focus on addressing issues that will decrease medical costs and increase the quality of life.
  • Develop positive relationships with clients, caregivers, providers, and other team members.
  • Complete assigned assessments and develop appropriate care planning.
  • Interact with peers in a collegial manner, supporting efforts to improve outcomes.
  • Managing physical health problems through the provision of education.
  • Connecting with other community services.
  • Identification of dietary concerns.
  • Home safety evaluations.
  • Safe medicine use.
  • Education on specific illnesses and treatments.
  • Stress management.
  • Emotional support.
  • The monitoring process of clients through home visits.
  • Compliance with HIPAA regulations.
  • Maintain accurate records of care coordination in the EMR.

SKILLS & QUALIFICATIONS

  • One (1) year of experience as an SSW is preferred.
  • Computer skills required: (Microsoft Office Suite Software).
  • Adapts to changes in policies, procedures, new techniques, & additional responsibilities.
  • Clean driving record.
  • Knowledge of HIPAA regulations.
  • Bilingual (English/Spanish) is preferred.


EDUCATION, LICENSES, & CERTIFICATIONS

  • High School Diploma or equivalent required.
  • Bachelor’s Degree in Social Work (BSW) is strongly preferred.
  • Minimum of 5 years of experience and/or training and/or a combination of education and experience can be considered.
  • Current and clear driver's license.

PHYSICAL DEMANDS & WORK ENVIRONMENT

  • Must be able to frequently drive long distances, which may require a two-hour radius of your designated county (travel time is paid and mileage reimbursement is available).
  • Occasionally required to stand.

  • Continually required to sit.
  • Occasionally required to climb, balance, bend, stoop, kneel, or crawl.
  • Continually required to talk or hear.
  • While performing the duties of this job, the noise level in the work environment is usually moderate.
  • The employee may occasionally lift and /or move more than 40 pounds.
