SummitWest Care is seeking a Full-Time Physical Therapy Assistant (PTA) to join us in caring for our clients. You will provide client therapy services to clients in their homes to help them improve and/or restore their independence and ability to perform daily living within your scope of practice.
Position Requirements:
- Performs treatment and caries out interventions to meet goals assigned by Physical Therapist.
- Provides Physical therapy services to client according to physician orders and regulations.
- Assures PT is notified in a timely manner of changes in client status.
- As identified by Physical Therapist, makes appropriate modifications to equipment/environment to meet client needs.
- Provides teaching and instruction to client/caregiver to promote physical and emotional health.
- Attends required staff meetings and case conferences.
- Contributes to the coordination of services through effective and timely communication with other members of the health care team.
- Maintains the caseload and agency productivity standards.
- Maintains clients' self-esteem, privacy and confidentiality of personal information.
- Assures client rights are protected and that all clients/caregivers are made aware of the client's rights and responsibilities.
- Initiates emergency procedures as appropriate.
- Clinical and administrative documentation adheres to agency, professional, and regulatory guidelines for timeliness and content.
- Performs other duties within scope of practice, as assigned by Sr. Director of Clinical Operation
Education and Experience:
- Associate's degree for Physical Therapy Assistant
- Currently licensed and in good standing in the State of Colorado
- Previous home care experience and Physical Therapy experience
