A mental health or behavioral health Case Manager serves as a residents safety net. In their role, Case Managers coordinate services and connect residents with resources that can help them with their challenges, whether they be psychiatric illness, substance abuse or other personal problems that impact their ability to function normally in everyday life.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Core duties and responsibilities include the following. Other duties may be assigned.
· Assists residents in developing goals in areas of need, and assists in developing treatment plans which are assessed regularly; conducts medication and mental status assessments as needed.
· Facilitates resident access to community resources, including locating recreational activities, housing, food, clothing, school programs, vocational/training opportunities, providers to teach life skills and relevant mental health services.
· Assists residents in developing natural resources and make contact with social support networks.
· Coordinates and monitors services, including comprehensive tracking of resident activities in relation to care plan.
· Follows up with referrals for services and regularly makes contact with referral sources for outcome statuses.
· Has regularly scheduled one on one contact with assigned residents.
· Identifies and provides emergency crisis services as necessary; makes immediate assessments and responds according to accepted crisis intervention methods and techniques.
· Documents all resident encounters and contract made on behalf of residents. Maintains comprehensive resident files.
· Provides reports and data to immediate supervisor as scheduled.

EDUCATION AND/OR EXPERIENCE:
- BS/BA Degree in a human services related discipline and 3-5 years’ experience working with applicable populations.
- Experience working with unhoused populations
