As a Senior Transitional Care Assistant, you play a vital role in guiding, educating, and advocating for clients, equipping them with the necessary knowledge for smooth care transitions. Acting as a bridge between clients and healthcare teams, you also handle administrative tasks and offer top-notch customer service.

What You Will Do

• Comprehend and discuss client-defined healthcare and functional goals, as well as expected post-discharge care needs, ensuring a seamless transition home or to another care setting.

• Collaborate closely with the Care team to monitor and document client progress accurately and in a timely manner.

• Conduct regular case record reviews and conferences with the Care team for assigned clients.

• Cultivate and maintain collaborative relationships with caregivers, hospital staff, physicians, and other healthcare professionals to facilitate effective care transitions.

• Record all interdisciplinary team interactions in the EMR system.

• Follow up with clients within expected timeframes according to established workflows.

Education

• Associate's degree in human services or a related field, or equivalent work experience, required

• Preferred: Bachelor's degree in human services or related field

Work Experience

• Minimum of two years of customer service experience in the medical field (e.g., hospital, long-term care, home care, medical office), required

• Adaptability to change and ability to thrive in a team-oriented environment, required
