Job description

Primary Work Functions Under general supervision, leads the planning, organizing, and implementation of NKFMs Participant Program Referrals Team including receiving, processing, triage, and outreach to all referrals. Leads a statewide team through systems design and management, ongoing program teams training and delivers reports and assists with data collection as appropriate. Working with Leadership, this position will also manage the systems and teams responsible for program materials distribution.

The Senior Program Coordinator will work with other programs as directed. Will receive other assignments when indicated. Major Job Duties and Responsibilities Encourages and models the NKFM Guiding Principles of integrity, caring, teamwork, innovation, and dedication by utilizing practices of The Collaborative Way.

Oversees incoming program referrals from various program partners. (60%) Manages online referral portals (EPIC, MiHIN, ReferralMD, secure fax) Manages program triage process, including designing collaborative systems with various program leads Manages various participant intake databases and works to find, develop, and execute efficiencies Supervises Program Referrals Team in their daily tasks of organizing, assigning, outreaching, and notetaking for each active referral in our databases. Works with the Analytics and Logistics Team to provide necessary data for program referral reporting, as needed.

Manages all participant program materials processes (20%) Supervises Program Coordinator Assistants to effectively implement program materials distribution. Oversees materials ordering and management to maintain proper inventory of program materials. Works with program leads to create and align efficient and effective distribution and tracking processes.

Works with identified partners (current and new) to develop referral relationships and to expand possible coverage for the program. (20%). General Duties As assigned, provide data entry, answer phones, complete reports, material development, etc.

Maintain daily calendar, report to meetings in a timely manner and complete all assignments. Plan and coordinate meetings with staff and community partners as directed. Provide leadership and be recognized by staff and community volunteers as a professional mentor and role model.

Recruit partners to participate in the NKFM programs as needed. Communicate effectively with supervisor, volunteers, staff, and community members to carry out the responsibilities of the job, promote cooperation within and between these groups. Integrates educational efforts, focusing on increasing awareness regarding the prevention of kidney disease and promoting programs that are available that improve the quality of life for people in low-income areas.

Other Assignments The NKFM is a dynamic organization that embraces change to support our mission. Staff members typically add responsibilities as new programming and funding opportunities arise that align with their skills and interests. As an emerging leader, the Program Manager will support NKFM operations and organizational development such as diversity, equity, and inclusion (DEI), organizational culture, hiring and retention, and development.
