Your Specific Responsibilities:
The incumbent is part of a global public health organization and is expected to support the Florida Department of Health in Clay County in its mission to protect, promote and improve the health of all people in Florida through integrated state, county and community efforts and its vision to be the Healthiest State in the Nation. Furthermore, the incumbent is expected to incorporate the agency values of Innovation, Collaboration, Accountability, Responsiveness and Excellence in all aspects of their work.
Disease Interviewing
The primary responsibility of this position includes acting as a community liaison for STD outreach/field work. The individual in this position will interview, re-interview, and conduct cluster interviews for all clients diagnosed with syphilis and/or HIV, their contacts, suspects, and associates and to update STARS. Performs all related case management. As determined by the County Public Health STD Program Manager, performs Pre and Post - Test Counseling for confidential/anonymous HIV test to clients. Conducts Gonorrhea and Chlamydia interviews with pregnant women. These diseases include but not limited to: STD/HIV and other reportable infectious diseases. Enter cases, field records, and document all follow-up into the STARS computer system in a timely and accurate manner. Field records should not be open more than 30 days from the initiation date. Meets timeliness and accuracy standards set by the STD program office. Complies with Health Insurance Portability and Accountability Act (HIPAA) and applicable Privacy Laws.
Community Outreach/Education
Conducts organized community outreach screenings and assists with program planning and special projects to include joint ventures with HIV/AIDS program. This includes educational presentations related to STDs and HIV/AIDS topics to professional and community groups as necessary. Distributes condoms through-out the community to businesses and individuals. Represents the department when needed by active participation in public health outreach activities and local HIV/AIDS planning group using county vehicle.
Field Investigations
Conducts field investigations to locate and successfully refer to examination/treatment those persons known to be infected or suspected of being at risk of infection. These diseases include but not limited to: STD/HIV/TB/Hep B and other reportable infectious diseases. Provides field blood or oral swab testing for syphilis and/or HIV contacts, suspects, associates, and other high-risk individuals. To accomplish their duties, Senior Human Services Program Analyst must: find locating information using person finding software such as LexisNexis, Department of Corrections databases, registries, and hospital/health care electronic medical records. Senior Human Services are responsible for utilizing proven social, behavioral, educational, and sociological tools to identify and overcome barriers to linkage and retention in medical care for HIV clients. Collects specimens by venipuncture, oral swabbing, and/or urine-based techniques for syphilis and/or HIV contacts, suspects, associates, and other high-risk individuals.
Accountability
Maintains 100% accountability by regular submission of the field accountability log, Clinical Report, Monthly Reports, Time, and Attendance through PeopleFirst for supervisor's approval. Recording of Daily Employee Activity Records (EARs) are critical to the Department's operational budget. Based on these requirements, all data must be recorded timely and accurately for compliance with programmatic and administrative guidelines, with no less than 95% compliancy for any given timeframe.
Must complete BLS education within 6 months of employment and renew education every 2 years as provided by the Department.
Performs all the other duties as required.
Required Knowledge, Skills, and Abilities:

  • Knowledge of HIPAA.
  • Ability to use of all Microsoft Office products.
  • Knowledge in 500/501 HIV Counseling and Testing.
  • Ability to conduct Phlebotomy related duties.
  • Ability to drive vehicle to complete job duties.
  • Ability to analyze data and client specific information.
  • Ability to evaluate and tabulate statistical information.
  • Ability to evaluate and document all assigned duties.
  • Ability to formulate reports using various work systems.
  • Ability to handle time management.
  • Ability to convey information to a single person and/or small to large groups.
  • Knowledge of computer software including applications and data entry skills.

Qualifications:
Minimum -
A bachelor's degree from an accredited college or university.
A valid driver's license and willingness to drive a county vehicle.
Must be willing to attend PASSPORT to Partner Services training within first 6 months of employment.
Must be willing to attend 500/501 training within first 12 months of employment. If you have a valid certificate, the certificate can be uploaded with your application.
Preferred -
At least 6 months of experience in developing and preparing reports and/or presentations.
Valid HIV/AIDS 501 Certification.
