Full Time or Part Time 6:00am-2:30pm/7:00am-2:00pm and Full Time or Part Time 2:00pm - 10:30pm. Requires at minimum every other weekend and every other holiday.

Qualifications
  1. Must have reliable transportation to and from work.
  2. Previous care giving experience preferred.
  3. Able to read, write, hear and speak the English language and follow oral and written instructions. Able to communicate effectively with tenants and staff.
  4. Background check passed.
  5. Must meet the requirements of Senior Caregiving tasks, as outlined in job-related responsibilities.
  6. Free from illegal use of drugs, and free from the use and effects of drugs and alcohol in the workplace.

Skills Required
  1. Communicates effectively, both written and orally.
  2. Must possess the ability to make independent decisions using sound critical thinking, follow instructions, and to accept guidance.
  3. Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing caregiver practices.

  5. Must have the ability to work productively in the absence of or with minimal supervision.

General Responsibilities
  1. Knowledge of and respect for the rights, dignity and individuality of each tenant in all interactions.
  2. Appreciates and maintains confidentiality of tenants and facility information.
  3. Demonstrates honesty and integrity at all times in the care and use of tenant and facility property.
  4. Ability to express self adequately and accurately in written and oral communication with staff, tenants, interdisciplinary team members, administration, and other professionals.
  5. Knowledge of emergency and disaster procedures of the facility, ability to understand and respond to written and/or oral interactions in case of an emergency.
  6. Demonstrate respect for co-workers by complying with facility policies on attendance, punctuality, and dress code. Able to arrive and begin work on time and to report for duty on all shifts and at the time of emergency.
  7. Working knowledge of and the ability to comply with facility policies and procedures for workplace safety, including infection control and bloodborne pathogen procedures, application of universal precautions, use of personal safety equipment, and handling of hazardous materials. Immediately reports any suspected abuse or neglect per facility policy and the Vulnerable Adult Law.
  8. Demonstrates ability to prioritize tasks/responsibilities and complete duties/projects within allotted time. Ability to respond to change productively and to handle additional tasks/projects within allotted time.
  9. Personally observes all assigned tenants to monitor their physical, medical and psychosocial status, updates the RN of any changes.
  10. Is continually aware of the condition of the unit regarding appointments, illness, calendar events, etc.
  11. Ability to carry out the essential functions of this job without posing risk of substantial harm to health and safety of self and others.
  12. Has an understanding that tenants with dementia and related disorders may become verbally or physically aggressive at times for differing reasons. Ability to apply a variety of approaches and to recognize and intervene before tenant becomes aggressive.

Job Related Responsibilities
1. Successful completion of all tasks as listed on the General Orientation for Home Care Employees Checklist as scheduled.
2. Nursing Services that May be Delegated to a Senior Caregiver (ULP)
  • Maintaining central storage of medications.
  • Providing medication reminders with daily or weekly pill containers.
  • Performing assistance with self-administration of medications and medication administration. Such medications include oral, eye drops/ointments, eardrops, oral/nasal inhalants, topical, transdermal, and PRN medications as directed.
  • Appropriate and safe techniques in personal hygiene and grooming including hair care, bathing, care of teeth, gums, and oral prosthetic devices; care and use of hearing aids; assistance with dressing, toileting and bathing.
  • Assistance with appropriate and safe techniques in transferring tenants; able to provide tenant transfers using stand-by, 1 or 2 staff assist. Safe assistance with tenant ambulation.
  • Other delegated tasks as assigned: Oxygen use/ service; Range of motion, exercise and positioning assistance; Respiratory support (nebulizers/ Cpap/ Bi-pap); Diabetic support including blood glucose monitoring; nail care; colostomy care; urine specimens; dressings/ dressing changes; ace wraps/ ted socks; skin care; alarms, including tabs/ motion sensor, door alarms; side rails; bowel and bladder control, devices and training programs, including incontinence care; catheter care; vital signs; feeding assistance (who may be at risk for choking); assistance with tenants during an illness; hot and cold packs.
  • Promptly contacts the RN to report tenant changes in condition and/or for advice when needed. Follows established shift routines regarding monitoring of vital signs, weights, administration of medications and/or treatments & documentation requirements.
  • Other tasks as deemed appropriate and as delegated by the RN.

3. Non-Delegated Tasks - provides cares to meet the physical and psychosocial needs of each tenant. This includes personal cares, rest, activity, comfort, nutrition, elimination, psychosocial, and spiritual needs, such as:
  • Preparing meals and assisting tenants to meals.
  • Providing skin care, including full or partial bathing or foot soaks, lotion, report skin breakdown to nurses.
  • Service Coordination-assist tenant to schedule appointments/transportation. Escort tenants to/from activities including spiritual gatherings on campus. Activity reminders, preparation and participation with tenants.
  • Spend 1:1 time with tenant in need of additional emotional support at times.
  • Observes tenants and reports to nurse if any additional tasks/services may be needed in a timely manner and per policy.
  • Updates families as instructed.
  • Follows individual care plan logs to provide the outlined services to individual tenants. Observes, records, and reports to a supervisor any changes in tenant status.
  • Other tasks as necessary, assigned, and delegated.
  1. Provides Laundry Services for each tenant, as included in their basic rental package, and additional as purchased by the tenant. Basic services include:
  • Changing and washing bed linens.
  • Washing and folding personal laundry.
  • Delivering clean and folded laundry to tenants.
  • Ironing, putting away clothes, and extra loads of laundry, as purchased
  • In Memory Care, the Senior Caregiver assists to make the bed daily.

  1. Provides Dietary Services to each tenant on daily basis, as included in their basic rental package. Services include:
  • Basic cooking and baking may be required in your work area.
  • Breakfast, Lunch, and Dinner are provided in the basic rental package. Provide mealtime reminders as directed.
  • Put out condiments, utensils, napkins, cups, glasses, coffee, and water on the tables; clear and wipe off tables (Dietary and Caregivers). Collect apartment trays if applicable.
  • Deliver meals to each tenant at their table (deliver to apartment if requested).
  • Assist to cut or butter food, etc. Orient to food on plate.
  • Wash and put away all dishes (Dietary and Caregivers).
  1. Provides Housekeeping Services (Along with housekeeping department) in each Commons Areas/Apt. of the Senior Housing Complex, as outlined on assignment sheets.
  • Empty Garbage in apt. daily
  • Straighten up areas after use and as needed
  • Plant care
  • Clean furnishings as needed
  • As needed, clean floors to include sweeping, vacuuming, damp/wet mopping, assure that appropriate caution / safety signs are properly displayed prior to performing such duties.
  1. Encourage social interaction of tenants. Responsible to know activities schedule and invite/remind tenants prior to the activity starting, assist as needed to escort to and from activities. Answer to team spirit calls to assist with activity escorts/needs. Responsible to perform activities on activity schedule as assigned by activity staff and in his/her absence. Responsible to have tenants ready for activities at assigned times.
  2. Plans and organizes workload efficiently. Responsible to watch assigned tenants. Ability to help team members and works as a team player.
  3. Effectively communicates verbally and nonverbally to ensure quality care and assistance is given.
  4. Attends meetings and in-services as scheduled.
  5. Assists in maintaining accurate records of tenants as assigned. Gives input into care plan and/or service plan of tenants.
  6. Promotes tenant rights and responsibilities including ensuring that each tenant is treated as an individual with privacy, dignity, and confidentiality maintained.
  7. Will participate in staff conferences/meetings, as required.
  8. Will maintain good housekeeping & infection control standards within assigned duty areas.
  9. Meets safety and infection control standards, per policy.
  10. Participates in staff education programs and attends at least 8 in-service hours per year. Will attend job-related meetings as required by supervisor. Will also attend mandatory training related to Alzheimer's disease.
  11. Other functions as assigned by the Director of Nursing or designee.

Physical / Intellectual Functions:
  1. Reasoning: able to solve problems individually or by consulting appropriate supervisors.
  2. Mathematical: able to perform calculations including fraction, decimals, and percentages.
  3. Communication & Comprehension: able to understand and follow policies and procedures that pertain to tasks, cares, procedures, and other duties that are to be performed in this position. Able to communicate effectively with tenants, staff and visitors.
  4. Attitude: Treats and ensures that tenants are treated with respect and dignity. Displays a positive and supportive attitude. Willing to fill in for others as needed, able to work beyond normal working hours when necessary. Willingly helps other staff in any department. Is a team player. Leads by setting a good example, embraces new staff and is encouraging and open in aiding the orientee in his/her learning process. Accountable for one's own actions. Accepts constructive criticism and is willing to try new ideas. Accepts feelings and behaviors displayed by tenants which differ from own feelings. Discusses the positive aspects of Annandale Care Center to co-workers and the community.
  5. Physical Demands: Safely transfers tenant weighing equivalent to 25-35 pounds. Frequently pushes or pulls tenants 300-600 feet with weight resistance equivalent to 50 pounds. Per Physical & Environmental Job Requirements: able to twist, stoop, bend, squat & kneel; lift and carry weight of up to 35 pounds; climb stairs; reach at or above shoulder level. Able to endure prolonged walking, sitting, standing, pulling, bending, stooping, balancing and lifting (of 0-35 pounds) movements.

Able to perform repetitive motions of wrists, hands, finger, pushing, pulling, grasping, reaching, and walking. Able to adequately hear tenants, staff, alarms, devices, etc. Able to appropriately identify tenants, call lights, unusual occurrences on unit, at a distance of 125 feet. Able to correctly differentiate between blood and other body fluids. Other sensory and communication activities essential include the ability to feel, smell and speak appropriately. Able to read small print in typed or written material. Possess sound reasoning skills; able to assist in solving practical problems. Concentrates with frequent interruptions, remembers multiple messages, information, and tasks during the regularly scheduled shift. Possesses necessary skills to continuously comprehend and communicate with tenants, co-workers and to respond to emergency alarms and other tenant needs.

6. Able to work beyond normal working hours when necessary (on-call, emergencies).
Environmental Job Demands / Working Conditions:
  • The Senior Caregiver is at significant risk for potential exposure to blood borne pathogens such as Hepatitis B, Human Immunodeficiency Virus (HIV), etc.
  • Potential for exposure to hazardous chemicals as identified by MSDS manual.
  • Potential for exposure to bodily injuries due to unpredictable behaviors of tenants.
  • Potential for exposure to infectious bodily fluids of tenants, staff or others. Potential for respiratory and chemical hazards.
  • Occasional exposure to microbial and other infectious agents in care of ill/dying tenants.
  • Occasional exposure to latex, plastic, vinyl, and/or materials which are used for personal protective equipment.
  • Reports occupational exposures to blood and body fluids, infectious materials, and hazardous chemicals in accordance with the facility protocol.
  • Annandale Care Center/ Centennial Villa Assisted Living provide nursing care 24 hours a day, 7 days a week. Shift lengths vary, most positions involve working every other weekend and holiday. The caregiver must remain on duty until the next shift arrives.
  • Uniforms are any colored scrub shirts and pants. Shoes should have good support, non-skid soles, and closed toes. Nametags must be worn at all times.
