Responsibilities:
Health Records:
Review all new entries, including PreK, HRS 680 forms and physical forms. Also, perform annual reviews on Kg and 7 grade for updates, compliance and accuracy.
Enter necessary data from forms into SIS. The Health Dept. remotely monitors for physical and immunization requirement compliance.
Create and maintain Medical and Religious immunization exemption lists.
Medications:
Medication administration and associated paperwork including: creation of a MAR; obtaining correct Physician authorization form, signature from parent; obtaining correct pharmacy labeled, unexpired medication with receipt co-signed by parent; and daily documentation. Teachers and key staff signature on forms when signing out meds. All physician authorizations received during the school year must be dated after June 1 of the year that school year begins and it is good through summer school when that school year ends.
Accurate medication inventory and counts are the responsibility of the School Nurse.
New Physician authorization forms are required for each new school year.
Medication Inventory:
Track expiration dates on all meds and request replacements from parents as needed. I have found that phone calls work best.
Maintain daily medication and complex medical procedure list (generate weekly and update as needed)
Maintain PRN medication list with student name, chronic health condition, medication, expiration date, teacher name and room number.
Chronic Health Conditions:
Maintain Chronic Health Condition (CHC) and High-Risk list with student name and CHC, teacher name and room number.
Provision of student-specific case management for CHC students. Track progress via Tracking Log.
Track the students on the CHC list for at least annual updates to medical history, including medications and recent events (done via parent questionnaires and/or updates).
Track the annual training provided to all staff working with students with CHCs. This includes first aid for CHC emergencies and administration of emergency meds as per the doctor order and Emergency care plan. Complete associated training forms. Update as needed for changes in staff and/or classrooms.
Development of an Individualized Health Care Plan for High-Risk students.
The above includes the care and management of all diabetic students including training of staff for CBI management, monthly insulin replacement, maintaining all supplies and replacement as needed, updating Diabetic Medical Management Plan as provided by the physician. Completion of training forms.
*Classroom lists can be obtained from Stacey Trozolino at the beginning of the year.
*Alpha lists can be printed from SIS.
Student Injury, Illness and ER Care:
Provision of illness assessment, first aid and emergency care to all ESE students ages 3 to 22. Provision of all medications and treatments ordered by their PCP. Exception: if there is no consent for School Health Services. If medications or treatments are to be given, a Consent for School Health Services must be on file. (Status can be found in SIS).
Staff Injury and ER care:
Provision of first aid and emergency care to staff who become ill (per Principal request) and/or are injured on the job. Completion of Incident Report for HR/Administration.
Documentation:
Documentation of Nursing Notes for all clinic visit encounters and confidential filing system of same.
Clinic records of withdrawn students are maintained confidentially and in an organized manner.
At the end of each school year, all records generated via logs are collected and stored confidentially.
Annual State Mandated Screenings:
Vision, Hearing, BMI and Scoliosis screenings are completed annually on students in Kindergarten, First, Third and Sixth grades. Screening requirements vary with grade level. The Health Dept. will loan you a Spot Vision Screener and Scoliometer to complete those screenings. Ellen Stern, SLP, will assist with hearing screening. They want this completed before Winter Break.
Results are provided to the Health Department via report forms they will provide.
Results are entered into SIS.
Letters with results are sent home to all parents.
Follow up and referral via phone calls to parents of students who did not pass.
At the end of the school year, the Annual Screenings report form with outcomes is submitted to the Health Dept.
CBI Medication:
Sign meds in and out for all students going on a CBI who have meds in the clinic. Medication must be signed out to trained staff. Training is provided up to the last minute, if needed. (Staff are to notify the nurse 2 or 3 days before CBIs so last minute training is usually not necessary).
AED/ER Bag:
Monthly checks and inventory of ER Bag and supplies as well as AEDs (3) and supplies are a state requirement.
Checklists are available.
Medical Supplies:
Maintain medical supplies and reorder as needed. This is done through Laurie Marino.
Clinic Logs:
Maintain and update Chronic Condition List
Maintain and update PRN medication list
Maintain and update Daily Medication Check off list.
The above lists are kept in the Medication Administration record, The CHC book, the medication cabinet and the ER bag and updated as needed.
(Keep Principals and key staff updated with any changes to the condition of students on this list).
Maintain 911 Log.
Maintain Case Management Tracking Log.
Maintain Daily Health Room Log. Use this form to log in all clinic events. You can pull many of the numbers needed for your weekly and monthly numbers to the Health Dept. from these logs.
Track medications given, complex medical procedures, consultations, first aid, nursing assessments, office visits, 911 calls, office visit outcomes, record reviews, training and IHCP development weekly.
Collect these numbers weekly and enter in the Monthly Service Report. Total all numbers at the end of the month and forward to the Health Dept.
At the end of the school year the Health Dept. will require an annual report. These above logs will assist you in collecting the data necessary for this report.
Maintain the Daily Refrigerator Temperature Log.
Resource:
Function as a resource to staff, parents and students for health-related concerns.
Communicate updates and student-specific concerns to Principals and Administration. Be sure to always keep your Principal updated on important matters.
