A reasonable estimate of the current range is $135,240-$182,972.
Position Summary:
As an essential part of our Primary Care Team, you will join a multidisciplinary team in a school-based health center setting to provide students with on-site access to comprehensive clinical services, including integrated primary medical, preventive, reproductive health, mental health care and a diverse array of health education and health promotion services.
Essential Functions:
Practice within the scope of the California Board of Registered Nursing and/or Physician's assisting practices and under the oversight of the Medical Director or designee, and within organizational guidelines.
- Obtain health history through patient / caregiver interview and chart review.
- Assess presenting illness and risk factors, family health history, review of systems, psychosocial / developmental histories, and cultural information.
- Perform appropriate physical examination and assessment.
- Order and perform appropriate diagnostic and screening tests and collect additional data for evaluation of illness / wellness.
- Diagnose and treat acute and chronic conditions.
- Develop and implement individual treatment plans, which may include internal and external referrals to specialty care services, diagnostic imaging facilities, and community agencies.
- Assist and instruct other health care employees in the management of patient care.
- Prescribe and dispense medications and monitor response to treatment.
- Perform minor procedures.
- Provide comprehensive family planning services which may include initiation of oral, injectable, and indwelling contraception (intrauterine devices and subdermal implants.)
- Provide primary care mental health services and appropriate referral to higher level of care when indicated.
- Maintain patient records in Electronic Health Record (EHR).
- Collaborate and coordinate patient care with the clinical care team, school staff, and community organizations serving the Sacramento Community.
- Ensure that patient records are kept confidential consistent with the Health Department policies and procedures and HIPAA standards.
- Monitor HIPAA compliance within the work environment.
- Comply with established policies and procedures, quality assurance, safety, environmental, infection control and comply with requirements of accreditation and regulatory agencies.

Minimum Qualifications:
- Current RN/NP or PA license with corresponding State Board of California
- Current certification with governing body
- Current active DEA registration or the ability to obtain upon hire
- BLS Certification
- Minimum of two years NP experience
- Strong communication, interpersonal, and triage skills
Preferred Qualifications
- Minimum of two years School Based Health or Adolescent Ambulatory Care experience
- Bilingual in English and Spanish.
