Job description

We are seeking a highly organized and detail-oriented scheduler/care coordinator to join our team Right at Home, In-Home Care & Assistance. Scheduler/Care Coordinator will work closely with our clinical team to ensure the smooth coordination of services for our clients. The ideal candidate will have excellent communication and problem-solving skills, be able to work independently and as part of a team, and have experience unmanaging schedules and coordinating care for clients in Home Care.
Responsibilities:
- Developing and managing master schedules for client care, including coordinating appointments, visits, and assessments
- Communicating with clients and their families to schedule services and ensure their needs are met
- Collaborating with our clinical team to ensure accurate records and timely communication of
client information
- Managing and maintaining client records in electronic medical records (EMR) systems - Coordinating with insurance companies and other third-party payers to ensure proper billing and reimbursement
- Providing excellent customer service to clients and their families, responding to inquiries and addressing any concerns
Requirements:
- High school diploma or equivalent; Bachelor's degree in healthcare or related field preferred
- Minimum of 2 years of experience in scheduling and care coordination in a healthcare or social services setting
- Excellent communication and problem-solving skills
- Ability to work independently and as part of a team
- Strong organizational and multitasking skills - Knowledge of electronic medical records (EMR) systems
- Basic office and computer skills and organizational abilities. Excellent interpersonal relations abilities. Excellent telephone skills
- Knowledge of common medical terminology.
- Able to work independently, demonstrating sound judgment.
- Read, write, speak, and understand English as needed for the job.
- Be available as required for on-call duty outside of normal office hours.

  • Works primarily out of the local office. 8a-5p/9a-4
  • Regular rate - $16 per hour/40-hr work week
  • After hours compensation based on time and day
  • Paid holidays, health insurance paid 50%
  • Quarterly bonuses based on growth

Compensation: $16.00 per hour
