```Duties:```
- Conduct intake assessments with clients to gather relevant information
- Assist clients in completing necessary paperwork and forms
- Maintain accurate and up-to-date client records
- Coordinate and schedule appointments for clients with appropriate staff members
- Provide information and resources to clients regarding available services and programs
- Collaborate with other team members to ensure seamless service delivery
- Follow established protocols and procedures for client intake and documentation
```Qualifications:```
- High school diploma or equivalent (Bachelor's degree preferred)
- Previous experience in a similar role or in a healthcare/social services setting is a plus
- Excellent communication and interpersonal skills
- Strong organizational and time management abilities
- Ability to handle confidential information with sensitivity and discretion
- Proficient computer skills, including knowledge of electronic medical record systems
- Ability to work independently as well as part of a team
