POSITION SUMMARY
Non-Medical Case Management Coordinator coordinates and ensures services focused on guiding and assisting in improving access to needed services. To establish a collaborative process that assesses, educates, plans, implements, coordinates, monitors and evaluates the options and services required to meet the client's health and human service needs. Involves assessment and basic care needs planning with the goal of independence for the client. With a goal of getting HIV-positive clients into care and maintaining compliance with doctors' appointments and medication compliance with aim of ending the HIV epidemic.
POSITION DUTIES

- Greets patients, families, and providers in a courteous, friendly, and professional
- Complete initial assessment of patient according to department Policies and Procedures and documents
- Directly support Patient Support Specialist (Peers) as they assist clients with psycho-social needs and maneuvering through the Ryan White system of services.
- Collaboration with professional and community groups to develop goals and to increase compliance to care.
- Helping clients with applications for all other resources available.
- Ensure clients have all necessary documents to meet eligibility requirements for BRTGA Part A services.
- Providing unbiased and ethical services.
- Responsible for monitoring compliance with program contracts, monitors budgets, expenditures, and billing processes of assigned program.
- Completes reports required by program manager in a timely
- Helping and empowering clients to develop and utilize independent living skills and strategies.
- Advocate on behalf of clients to decrease service gaps and remove barriers to services.
- Provide for a face-to-face or phone contact, and a home visit, as determined by client needs.
- Provide concise explanation during enrollment process of all services available and what client can expect once enrolled.
- Utilize HRSA acuity scale for level of need.
- Keep record of all clients enrolled and or assessed for Case Management.
- Enter required data into CareWare appropriately
- Refer client out to another agency if services can not be provided here at CSMD.
- To lead or provide consultative role in support group meetings for HIV-infected individuals
- To interact with other team members to facilitate productive and efficient team functioning.
- Provides patient and/or family, significant teaching as opportunities arises or as need is
- Explain procedures using correct terminology and notifying patients about their
- Ability to educate and relay information to patients in a clear and concise manner.
- Develop, evaluate, maintain, and promote the use of effective patient education materials in the clinic and the community at large.
- Initiates multi-disciplinary referrals as needed and does appropriate follow-up.

- Communicates with RW Care Team on No-show appointments.
- Will participate in Patient Case
- Will assist Program Manager and/ or RW Clinical Director as needed with CQI
- Will attend RW departmental
- Responsible for communication and referrals to outside Aids Service Organizations and community organizations
- Perform Rapid HIV testing and counseling.

REQUIREMENTS
Associate or bachelor's degree in a healthcare-related field.
- Current CPR certification is required
- Experience in HIV/STI
- Experience in collaboration with interdisciplinary healthcare
- Experience in ambulatory care setting
- Required to be able to successfully complete established competencies for the position within designated probationary period.
- Effective communication skills: both oral and written.
- Computer skills needed.
- Ability to work with others within a team to ensure quality patient care.
- S/He should have strong problem-solving skills. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
- Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule
