POSITION SUMMARY

Non-Medical Case Management Coordinator coordinates and ensures services focused on guiding and assisting in improving access to needed services. To establish a collaborative process that assesses, educates, plans, implements, coordinates, monitors and evaluates the options and services required to meet the client's health and human service needs. Involves assessment and basic care needs planning with the goal of independence for the client. With a goal of getting HIV-positive clients into care and maintaining compliance with doctors' appointments and medication compliance with aim of ending the HIV epidemic.


POSITION DUTIES

  1. Knowledge and understanding of Employee
  2. Carry out the meaning of the CSMD Mission and Vision statements.
  3. Practice a culture of safety to reduce or prevent risk of injury, claims, loss, or liability by utilizing the Risk Management and Infection Control Plan.
  4. Perform responsibilities with a high-quality standard to yield improved compliance, quality, and patient outcome measures by adhering to the Quality Improvement Plan.
  5. Be present and ready for work as
  6. Attend in-service trainings, departmental meetings, and community
  7. Work cohesively with team
  8. Greets patients, families, and providers in a courteous, friendly, and professional
  9. Complete initial assessment of patient according to department Policies and Procedures and documents
  10. Directly support Patient Support Specialist (Peers) as they assist clients with psycho-social needs and maneuvering through the Ryan White system of services.
  11. Collaboration with professional and community groups to develop goals and to increase compliance to care.
  12. Helping clients with applications for all other resources available.
  13. Ensure clients have all necessary documents to meet eligibility requirements for BRTGA Part A services.
  14. Providing unbiased and ethical services.
  15. Responsible for monitoring compliance with program contracts, monitors budgets, expenditures, and billing processes of assigned program.
  16. Completes reports required by program manager in a timely
  17. Helping and empowering clients to develop and utilize independent living skills and strategies.
  18. Advocate on behalf of clients to decrease service gaps and remove barriers to services.
  19. Provide for a face-to-face or phone contact, and a home visit, as determined by client needs.
  20. Provide concise explanation during enrollment process of all services available and what client can expect once enrolled.
  21. Utilize HRSA acuity scale for level of need.
  22. Keep record of all clients enrolled and or assessed for Case Management.
  23. Enter required data into CareWare appropriately
  24. Refer client out to another agency if services can not be provided here at CSMD.
  25. To lead or provide consultative role in support group meetings for HIV-infected individuals
  26. To interact with other team members to facilitate productive and efficient team functioning.
  27. Provides patient and/or family, significant teaching as opportunities arises or as need is
  28. Explain procedures using correct terminology and notifying patients about their
  29. Ability to educate and relay information to patients in a clear and concise manner.
  30. Develop, evaluate, maintain, and promote the use of effective patient education materials in the clinic and the community at large.
  31. Initiates multi-disciplinary referrals as needed and does appropriate follow-up.
  32. Assures informed consent is

  34. Attends general orientation.
  35. Identifies unsafe conditions and intervenes in a timely
  36. Responds to all emergency codes and
  37. Follows Standard precautions by utilizing appropriate Personal Protective Equipment.
  38. Demonstrates knowledge of Policies and Procedures
  39. Maintains open communication with all team
  40. Maintains adaptability, supports teamwork, and resolves
  41. Reads written communication provided on unit (meeting minutes, memos).
  42. Follows chain of command with problems and
  43. Accepts additional assignments willingly; and assists others as required to meet quality service and productivity expectations.
  44. Organize and participate in community activities, such as health fairs, school projects,
  45. Maintains appropriate documentation of all tasks.
  46. Communicates with RW Care Team on No-show appointments.
  47. Will participate in Patient Case
  48. Will assist Program Manager and/ or RW Clinical Director as needed with CQI
  49. Will attend RW departmental
  50. Responsible for communication and referrals to outside Aids Service Organizations and community organizations
  51. Perform Rapid HIV testing and counseling.

REQUIREMENTS

  1. Associate or bachelor's degree in a healthcare-related field.

  2. Current CPR certification is required
  3. Experience in HIV/STI
  4. Experience in collaboration with interdisciplinary healthcare
  5. Experience in ambulatory care setting
  6. Required to be able to successfully complete established competencies for the position within designated probationary period.
  7. Effective communication skills: both oral and written.
  8. Computer skills needed.
  9. Ability to work with others within a team to ensure quality patient care.
  10. S/He should have strong problem-solving skills. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  11. Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule
