Rn/Lpn Chronic Care Managment

Job Overview:
We are seeking a dedicated and experienced Chronic Care Management (CCM) Nurse to join our team. The CCM Nurse is responsible for providing comprehensive, coordinated care to patients with multiple chronic conditions. Responsibilities:
-Develop personalized care plans for patients with chronic conditions in collaboration with the healthcare team.

Provide patient-centered care, including regular monitoring of patients' health status, medication management, and adjustment of care plans as necessary.

Coordinate care with other healthcare professionals, ensuring timely communication and collaboration.

Educate patients and caregivers on disease management strategies, medication use, and lifestyle modifications.

Advocate for patients' needs and preferences.

Accurately and timely document patient interactions, interventions, and progress in the electronic health record system.

Participate in continuous professional development and training to stay updated on best practices in Chronic Care management.

Requirements:
1. Nurse (RN/LPN) with a valid nursing license in the state of Illinois interested in serving patients remotely.

2. Minimum of 1 years of nursing experience, preferably in chronic disease management or a related field. (Preferred not mandatory)

3. Excellent communication and interpersonal skills.

4. Strong clinical skills with the ability to work independently.

5. Knowledge of Medicare's CCM program guidelines and requirements.

6. Proficiency in electronic health record systems.

7. Ability to work collaboratively with a team and manage multiple priorities and reliably meet deadlines.

8. Commitment to patient-centered care and improving health outcomes.

9. Detail oriented, ability to multitask, and highly organized.
