We are looking for a Full-Time Registered Nurse (RN) to work as a Triage/Patient Call Center (PCC)Staff Trainer/Lead.
As a Patient Call Center (PCC) Trainer RN you are responsible for providing training to new staff, receiving patient phone calls, and following the practice or protocol while gathering pertinent data regarding the reason for the call.
Responsibilities:
- Collects information regarding patient’s general condition via telephone, particularly the acutely ill patient, and ensures that needs are met by use of TPB and/or licensed practitioners orders
- Documents all activities/interventions, patient/family responses, medication dispensed or prescribed, hospital data, etc., in the medical record as described by patient via telephone
- Responsible for greeting patients by answering multiple phone calls for all sites/locations in a timely and polite manner
- Efficiently uses an EMR system to document patient's records
- Per TPB and/or Voyage Healthcare Provider Approved Standing Orders:
- Describes or explains what can and will be done for the patient, i.e., speak with provider, schedule same-day appointment
- Coordinates patient care by ensuring appointments are scheduled based on urgency of phone call
- Promoted wellness by providing patient education materials, communicating provider's advice and instructions following clinic-specific protocols
- Providers information regarding diagnostic procedures, medications, nutrition and maintenance of health and wellness per the primary care provider's recommendation
- Relays information obtained from other professional callers, i.e. pharmacies, long-term care facilities, home care, hospitals, etc.
- Sends written messages to clinic providers for medical advice, prescriptions, etc.
- Completes accurate, clear, and thorough documentation on all telephone encounters
- Maintains confidentiality in matters regarding the clinic, its patients, and employees
- Responsible for “call-backs” pertaining to messages sent, labs and diagnostic imagining
- Prepares letters and orders for the Voyage Healthcare providers to sign
- Scans letters and form documents into the patient’s EMR
- Complies with procedures and policies to ensure a safe work environment and compliance with OSHA standards
- Performs other related duties as assigned
Requirements:
- Graduate of an accredited school of nursing required
- Registered Nurse (RN) current licensure as a registered nurse in the State of MN required.
- CPR Certification (must be obtained within 30 days of employment)
- Minimum 1 year experience preferred
- Excellent communication and interpersonal skills
- Ability to undertake a variety of diverse care tasks
