RN: Resident Care Coordinator

Full Time - Days

**Pay Rate up to $86,000/year; based on experience**

Requirements for an RN: Resident Care Coordinator:

  • Registered Nurse in the State of Ohio (BSN preferred).
  • Experience in Assisted Living, dementia, health and wellness.
  • Excellent managerial and supervisory skills.
  • Understands and communicates job-related information and directions in English.
  • Experience leading a neighborhood in a Long Term Care setting.
  • Experience leading and supervising clinical staff members.

Responsibilities for an RN: Resident Care Coordinator:

  • Assume 24 hour administrative authority, responsibility and accountability for clinical functions, activities and education of the Resident Care Staff at Jennings of Brecksville. Provide direct daily supervision of staff. Develop and train staff as necessary. Provide coaching and corrective action as necessary.
  • Assume accountability for the development, organization and implementation of approved policies and procedures
  • Assume responsibility for the development of Resident Care objectives and performance standards of nursing practice for nursing staff.
  • Obtain shift report from nurse on duty and record sufficient information to implement appropriate follow up action as necessary in relation to Resident Care needs.
  • Promote a social and residential model living environment.
  • Evaluate Resident Care as related to individualized resident needs, family involvement, and the physician's plan of care for the resident.
  • Conduct daily rounds and take corrective measures or report issues to the Administrator as necessary.
  • Identify special nursing problems and emergency situations quickly, and initiate immediate life saving measures in the absence of a physician.
  • Utilize personnel effectively to provide quality Resident Care.
  • Coordinate and report the investigation of incidents and accidents. Complete incident reports and investigation by the end of the shift on which the incident occurs. Report all incidents to Administrator and/or Director of Nursing; notify family or responsible party of changes in condition.
  • Receive and investigate complaints from residents/families and staff.
  • Participate in the interviewing, selecting, recruiting, and hiring process for clinical care staff. Direct efforts at maintaining staff and minimizing turnover.
  • Coordinate daily Resident Care provided in Jennings at Brecksville.Serve as liaison between physician, resident, family and staff. Listen to resident, family, or physician complaints and initiate corrective action and/or report special problems to Administrator and/or Director of Nursing.
  • Maintain an atmosphere which promotes growth, standards of care, clinical competence, philosophy, and mission.
  • Conduct team meetings for purposes of planning, coordinating, and implementing Resident Care policies and procedures and promoting quality of life for residents.
  • Assume responsibility for Resident Care compliance to federal, state, and local regulations.
  • Consistently operate Resident Care services within the budget guidelines established.
  • Coordinate allocation of Resident Care equipment and supplies within the facility and establish guidelines for efficient, economical utilization of supplies and equipment.
  • Assess equipment and supply needs and make recommendations to Administrator. Coordinate the ordering of needed supplies, to ensure adequate on hand inventory at all times.
  • Direct and supervise scheduling of staff within established guidelines for allocation of Resident Care personnel on duty.
  • Prepare written staff performance evaluations and initiate recommendations for salary increase, promotions, or terminations to Human Resources in a timely manner.
  • Participate in committees, both within the facility and outside community, i.e. Quality Assurance, Leading Age Ohio, and other related committees. Coordinate the Resident Care quality assurance and improvement process.
