Responsibilities for a Resident Care Coordinator Nurse:
• Assume responsibility and accountability for clinical functions, activities, and education of the Resident Care Partners. Provide direct daily supervision of staff. Develop and train staff as necessary. Provide coaching and corrective action as necessary.
• Assume accountability for the development, organization and implementation of approved policies and procedures.
• Assume responsibility for the development of Resident Care objectives and performance standards of nursing practice for nursing staff.
• Obtain shift reports from nurses on duty and record sufficient information to implement appropriate follow up action as necessary in relation to Resident Care needs with the assistance of the Wellness Coordinator.
• Help promote a social and residential model living environment.
• Evaluate Resident Care as related to individualized resident needs, family involvement, and the physician's plan of care for the resident.
• Conduct daily rounds and take corrective measures or report issues to the Administrator as necessary.
• Identify special nursing problems and emergency situations quickly and follow proper procedure.
• Coordinate and report the investigation of incidents and accidents. Complete incident reports and investigation by the end of the shift on which the incident occurs. Report all incidents to Administrator and/Wellness Coordinator; notify family or responsible party of changes in condition.
• Receive and investigate complaints from residents/families and staff.
• Participate in the interviewing, selecting, recruiting, and hiring process for clinical care staff. Direct efforts at maintaining staff and minimizing turnover.
• Maintain an atmosphere which promotes growth, standards of care, clinical competence, philosophy, and mission.
• Ensure that all federal, state, and local regulations are being adherd too.
• Participate in committees, both within the facility and outside community, i.e. Quality Assurance, Leading Age Ohio, and other related committees. Coordinate the Resident Care quality assurance and improvement process.
Responsibilities:
- Must be licensed as a Nurse (LPN) or (RN) by the state
- Skilled at critical thinking skills
- Can foster good resident and family relationships.
- Ability to effectively collaborate within a team environment.
- A passion for working with elderly residents.
- Experience in a long-term or assisted living facility a plus, but not required.
