RN - Quality Assurance/Performance Improvement (QAPI) Nurse

UP TO $5,000 SIGN-ON BONUS

The Quality Assurance/Performance Improvement Nurse has primary responsibility to review OASIS Assessments and other medical record documents and claims to assess for accuracy of service planning and quality of care provided, and to assess whether these services have met practice and ethical standard requirements and State and Federal Guidelines.

Minimum Qualifications:

Education: COS-C or similar preferred

Experience: At least 2 years Clinical experience in Home Health Care in a Medicare Certified Agency

Licensure: RN

Essential Job Functions:

• Provides the care team with clinical expertise, best practices and guidance in care planning and documentation requirements.
• Review documentation in the Electronic Medical Record for appropriateness and completeness including Plans of Care, Verbal Orders, Discharge Summaries
• Review documentation for visit utilization.
• Gather data, statistics, and reporting for QA program.
• Review OASIS and ensure optimal documentation provided timely to affiliate coders and reviewers for fiscal responsibility.
• Perform QA Audits
• Participate in Care Coordination conferences as needed.
• Other tasks and responsibilities as assigned by Clinical Director or Administrator
• Participates in On Call Process
