The RN performs as a professional nurse in the area of patient care and education, working closely with clinic prescribers and under the supervision of Behavioral Health Manager. The RN utilizes the nursing process (assessment, planning, intervention, and evaluation) in all patient encounters. Performs the duties of a registered nurse in accordance with the Nursing Code of Ethics, The Nurse Practice Act of the State of Wisconsin, WMH, and Nursing policies and procedures.
Responsibilities include:
- Patient Education: Establish teaching protocols for patient/family education and general public information. Performs educational assessment and patient/family teaching whenever needed and communicates instructions to patients/families that are clear and concise. Instructions are geared to the level of a pediatric, adolescent, adult, and or geriatric patient’s level of understanding. Documents patient/family teaching efforts and comprehension in Electronic Health Record.
- Information gathering/sharing:
- Reviews health history, addresses preventative maintenance issues as related to the pediatric, adolescent, adult, and geriatric patient.
- Documents accurate and complete collection of data as appropriate to patient contact.
- Assists medical provider with processing of diagnostic results bringing significant values to the providers’ attention. Has knowledge of tests and procedures and their significance as related to patient care. Contacts patients with test results and follow-up activities according to clinic practice.
- Triage/Screening: handles telephone calls related to patient care. Uses appropriate triage guidelines and protocols to manage patients care. Obtains appropriate information with attention to detail and accuracy (i.e. using direction of symptom related questions). Nursing assessment utilizing theory and judgment, and patient information. Implements appropriate care utilizing nursing assessment and provider preferences.
- Patient care duties: patient vitals, nursing assessment/intervention, GeneSite testing, providing nursing education for patients related to their symptoms/diagnosis/treatment plan/medication.
- Documentation/Communication: Documentation/charting via electronic health record is completed in a thorough and timely manner. Assists the prescriber in completion of various medical forms, (i.e. return to work, school forms, DOT, worker’s compensation, insurance prior authorization, etc.) Communicates and/or completes provider orders to outside sources (i.e. pharmacy, other clinics, hospital, nursing homes, etc. Screen calls for prescribers, manages refill requests and complete refills to pharmacies.
- Insurance: Works collaboratively with insurance companies to initiate referrals, and or obtain approvals, prior-authorizations for medical care and treatment.
- Referral/Resources: Works collaboratively with clinicians and staff both in and outside out clinic to assist with resources and facilitating referrals to higher levels of care when clinically appropriate.
- Performs other duties as assigned.
- Completes department-specific orientation and annual competencies for this position.
Requirements for this position include:
- Associates Degree; Bachelors preferred
- Demonstrates nursing experience in the field of mental health required. One year of experience in a hospital or clinic preferred.
- Ability to effectively communicate, verbally, orally and in writing, in person and telephonically, with co-workers, physicians/nurse practitioners, patients and patients families, must interact well with co-workers. Comfortable with computers and documenting in electronic health records.
- Required: Current Wisconsin Nursing License (RN).
Experience
Required- Demonstrates nursing experience in the field of mental health with one year in a hospital or clinic setting.
