Job description

The Bachelor prepared Registered Nurse plans, organizes, manages, and controls resources to achieve goals of Home Health Services. Provides supervision and oversight of professional nursing staff, all Certified Home Health Aides (CHHA) and Certified Nursing Assistants (CNA) including CHHA services provided to the participants in the home, community, or PACE Center. Oversees related and associated functions including scheduling, communication, follow-up, and on-call. Manages day-to-day clinical operations and ensures provision of high-quality care while interfacing and collaborating with staff, members of the management team, the community, and the Interdisciplinary Team (IDT). Provides administrative and managerial back-up to the Director of Clinical Services as needed.


  • Evaluates, develops, mentors, coaches, counsels, and disciplines department staff. Supports other personnel from other departments through coaching and mentoring to help achieve optimum standard of excellence. Conducts staff meetings regularly. Identifies staff education needs. Provides education and oversight regularly based on departmental needs. Attends and participates in scheduled training, in-service training, mandatory annual in-service training and educational classes as required and needed.
  • Actively participates as a member of the interdisciplinary team (IDT) in ongoing participant assessment and monitoring. Attends care planning meetings to report personal care services, patient self-care abilities, and changes noted in participant condition.
  • Evaluates and communicates on-going staffing needs. Makes recommendations to the Director of Clinical Services regarding staffing solutions and participates in interviewing and onboarding new employees to include development of the orientation schedule, oversight of the orientation process and competency review.
  • Ensures completion of competencies for RNs, CHHAs, and CNAs at the time of onboarding, annually and as indicated. Coordinates scheduled trainings as needed or required.
  • Oversees the coordination of staffing to meet day center needs in partnership with Day Center Manager. Provides clinical direction or assistance as requested or needed in the Day Center, clinic or home setting.
  • Compiles and maintains inventory and par levels for individual Participant (Ppt) supplies. Performs all aspects of supply procurement and maintains established inventory par levels for all ppts and Center usage. Assists other departments as needed with supply ordering.
  • Utilizes resources to meet department budget through oversight for department hours and schedules, payroll, mileage accuracy, and management of overtime.
  • Oversees the care provided to ensure participants’ safety and adherence to the care plan. Monitors that care is consistent with the organization’s policies and procedures and regulations.
  • Performs random supervisions for all CHHAs and CNAs to evaluate and monitor care provided. Educates staff to maintain the PACE participant independence by encouraging them to conduct daily living activities, including toileting, showers, eating, and grooming. Provides education on how to assist in the maintenance of physical activity, exercise, and walking.
  • Monitors participant safety by identifying safety hazards, emergencies, and care needs and staff safety related to patient care by identifying safety hazards, emergencies, and changes in care needs. Initiates corrective action as appropriate. Reports identified areas to the IDT to reach resolution when pertinent.
  • Reports identified discrepancies to the IDT for resolution. Documents services provided, reassessments of changing needs, and conversations with participants and families in the electronic medical record. Maintains strict confidentiality of all participant information.
  • Educates CHHAs, CNAs and other staff to identify signs and symptoms that should be reported to IDT indicating physical, emotional, behavioral change, skin integrity changes, bowel and bladder status, food and fluid intake, or changes in ambulation.
  • Maintains communication with participants and caregivers. Assists with filing and resolving grievances, service delivery requests (SDRs), and appeals in a timely manner. Reports issues with DME, refers to the appropriate resource, and resolves day-to-day operational problems reported by staff, participants, family, or other caregivers. Escalates significant or unresolved concerns to Director of Clinical Services or designee in a timely manner.
  • Collaborates with other departments to oversee and coordinate CHHAs and CNAs needed for medical, dental, optometry, and other outside appointments. Collaborates with IDT to determine the needs of participant related to appointments.
  • Develops and oversights review of center care plan and development of Restorative Nursing Plan.
  • Assures compliance with infection control, personal protective equipment, hazardous material handling, and fire safety standards. Reports any occupational exposures to Director Clinical Services or Senior Management in a timely manner.
  • Maintains oversight for complete and accurate CHHA and CNA schedules, including the amount, frequency, and service duration that the IDT has identified in the care plan. Records all scheduling changes, updates, and deletions on the schedule daily. Oversees service delivery and monitor participant satisfaction.
  • Works collaboratively with Director of Clinical Services. Provides administrative and managerial back-up as needed. Ensures all professional staff has current licensures and CPR certifications. Coordinates renewals as needed.
  • Provides on-call responsibilities after hours to assist with scheduling coverage, callouts, staff replacements, and troubleshoots problems. Escalates issues to on-call nurse or administrator on call as appropriate.
  • Participates in Quality Program, Elder Council, and other leadership meetings as assigned.

  • BSN required. ASN plus 7 years of experience in lieu of BSN.
  • Five years relevant RN experience.

  • Anticipated Occupational Exposure Risks Include the following: Bloodborne Pathogens , Chemical , Airborne Communicable Disease , Extreme Temperatures , Radiation , Extreme Noise Levels , Dust/Particulate Matter
