Job description

This position is responsible for providing primary nursing services to patients and their families in the Inpatient Center(s).
QUALIFICATION REQUIREMENTS

  • Graduate of an accredited school of registered nursing; BSN preferred.
  • Currently licensed as an RN through the Ohio Board of Nursing.
  • Minimum 1-2 years of clinical experience, with 1 year of experience in an acute setting preferred.
  • Has working experience charting in an EMR system, Netsmart system preferred.
  • Possesses clinical expertise related to critical thinking, assessment, problem-solving, and technical skills.
  • Possesses the ability to provide basic and skilled nursing care, set priorities of care, and appropriately assess limits of intervention.
  • Possesses strong verbal and written communication skills; demonstrates ability to communicate effectively with patients, families, and team members regarding care issues.
  • Has the ability to work independently and within an interdisciplinary team.
  • Demonstrates strong organizational, time management, and prioritization skills.
  • Must be CPR certified according to the American Heart Association CPR standards
  • Understands the hospice philosophy, principles of death/dying, concepts of pain/symptom management, and adult teaching/learning principles.

ESSENTIAL JOB RESPONSIBILITES
  • Implements the complete nursing process for assigned patients and their families, including:
    -- follows the plan of treatment and necessary revisions
    -- provides hands on patient care, medications, services, and prescribed treatments including those requiring professional nursing skills
    -- initiates appropriate referrals to other health disciplines and to other agencies for needed services
    -- teaches appropriate care, treatment, information and health practices to patients, families, caretaker(s), and others
    -- supervises and evaluates patients' self-care, care given by family, and care given by LPNs, hospice aides, and volunteers
    -- discharges patients according to agency policies
    -- Obtains and processes physician orders
    -- Maintains a safe environment for patients and uses equipment safely and properly
  • Coordinates services to maintain continuity of patient care and/or participate in appropriate clinical and interdisciplinary team conference.
  • Provides collaborative oversight of LPNs assigned to patient care as needed.
  • Instructs Hospice Aides assigned to patient care as needed.
  • Fulfills responsibilities of a Hospice Aide as needed.
  • Prepares, maintains, and dismisses clinical records in a timely manner according to agency policy.
  • Communicates and cooperates with other agency personnel to achieve patient care and agency requirements.
  • Participates in developing and maintaining expected standards of quality and quantity of patient care, including participating in record review.
  • Participates in selected public relation activities.
  • Provides field experience for affiliating students and new Hospice employees.
  • Maintains professional nursing standards.
  • Actively participates in the hospice quality improvement program, including identifying opportunities for improvement, problem solving and implementing recommendations.
  • Participates in in-services or other continuing education to meet own needs and agency requirements.
  • Responsible to learn and utilize technology made available by the agency to increase efficiency, communication and performance of duties.
  • Attends staff meetings and completes all mandatory training.
  • Holds in strictest confidence all patient information and discloses information and data only to persons authorized by Hospice.
  • Performs other duties as assigned.

Registered Nurses may have exposure to infectious diseases.
