MUST HAVE HOME HEALTH EXPERIENCE - This is a visitng Home Health nurse / case manager role. This is not for inpatient care.
Experience/Knowledge/Skills: This position requires a minimum of one year experience in acute care or community setting. Home Health experience preferred. Current licensure as a California RN, must obtain Oregon licensure within 6 months of hire; BLS - Health care provider; Valid CA Drivers license, access to reliable auto with proof of insurance. Must have demonstrated knowledge of the nursing process and community health care. Must have a working knowledge of applicable legal, regulatory and accrediting standards. Demonstrates knowledge of the concepts of death and dying, utilizing the AIM/End of life philosophy. Requires strong patient assessment skills as well as patient/caregiver education skills. Must possess basic computer literacy skills. Requires excellent organization, prioritization and communication skills. Requires effective English verbal and written communication skills. Must work well independently, without significant supervision or direction. Skilled in the following clinical interventions: central venous line management, pain management , wound management including NPWT devices, venipuncture, blood glucose monitoring and management of related medications and diet, coagulation testing and management of related medications and diet, bladder catheter insertion. Competent in functions of Case Management, including effective delegation skills. Sees 4-5 visits per eight hour day, including all Case Management duties (order follow-up, completes IDT case notes, POC's reviewed/finalized timely, etc.) Medical/Technical Skills: Demonstrates understanding of patho-physiology, rationale for treatment and educational factors for a wide variety of disease processes and body systems. Manages IV medications and sites, including infiltration, titration of medication and assessment with corresponding intervention. Manages IV equipment per nursing standards. Demonstrates knowledge of growth and development as evidenced by attendance and participation in in-services relevant to age specific criteria annually. Maintains patient and personnel safety from acquired infections by adhering to Universal Precautions 100% of the time. Professional Growth and Development: Accepts responsibility for own actions without excuses. Takes a proactive approach to problem resolution. Open to receiving and giving constructive criticism and listens to others. Maintains and updates own knowledge and skills as evidenced by participating in continuing education and in-services annually. Documents in Education Log. Attends, without urging, in-services to update technical skills and supports peer attendance at in-services. Sets own goals for professional growth and development and communicates these with Clinical Supervisor. Must complete all monthly competency classes as indicated by education log. Participates in all Performance Improvement projects annually.
