We service the following areas: Caroline County, Charles City County, Essex County, Hanover County, Henrico County, James City County King and Queen County, King William County, Middlesex County, New Kent County, Richmond County, and Richmond City.
Position
We are looking for a RN to join our Home Health team. This position will travel to the above localities to provide skilled nursing services to patients in their homes. In addition to direct patient care, you will be responsible for the case management of your assigned patients, creating patient-specific plans of care, and coordinating with interdisciplinary care team members to ensure top quality care for our patients.
Primary Duties and Responsibilities:
- Complete initial and ongoing comprehensive assessments of patient
- Initiate patient plan of care and reevaluate as necessary
- Perform supervisory visits on LPNs that also care for your patients
- Following a Physician approved plan of care, provide direct patient care and identify the physical, psychosocial, and environmental needs of patients.
- Assesses patient/caregiver’s knowledge of disease process, treatment regimen and/or medication administration. Provides basic and specialized teaching as ordered in the Plan of Care.
- Communicates significant findings, problems, or changes in the patient’s condition to the physician, Clinical Manager, and care team in a timely manner
- Documents all findings, Interventions, and patient response in EMR
- Identifies and recommends other disciplines needed for quality care of the patient.
- Advocates client’s needs with physicians and ancillary providers.
- Skills performed (but not limited to): Wound Care, Trach Care, G/J-tube Management, Post-operative Care, Foley Management, Subcutaneous Injections, lab draws, PICC/port care, management of drains
Knowledge, Skills and Ability Qualifications:
- Graduation from an Accredited school of nursing.
- Current licensure in good standing as a registered nurse in the Commonwealth of Virginia or eligible compact state licensure
- Two years minimum of experience in a med/surg environment or doctors’ office setting
- Valid CPR certification
- Valid Virginia driver’s license, acceptable driving record, and reliable transportation with insurance
- Ability to think and work independently and with direction.
- STRONG assessment skills
- Ability to communicate with staff members in a fast-paced and stressful environment.
- Strong computer and documentation skills
Other Requirements:
