Job Description
- Provide skilled nursing services to agency patients.
- Provide assessment of newly referred patients, assess health needs and eligibility for home care.
- Provide patient and family teaching prior to discharge and on an on-going basis.
- Develop and implement care plans in conjunction with patient’s authorized medical practitioner.
- Reassess patient needs for services on a continual basis and discuss any changes in the plan of treatment with the patient physician/family.
- Prepare written documentation and summaries of the patient’s progress as needed for the continuation of home health services.
- Plan, facilitate, and document patient’s discharge.
- Provide supervision and training of PCA/HHA staff
- Assist with the on-call rotation.
- NYS Licensed Registered Professional Nurse.
- Minimum of Associate's Degree required.
- One year Nursing experience required.
- One year home care experience a plus.
