Rn Case Mgr Emergency Department

GENERAL SUMMARY:
Provides case management for patients in the acute care setting. In collaboration with the physician and other health care team members, the RN Case Manager coordinates the patient's care to achieve optimal patient outcomes.
RN Case Manager Emergency Department
Location: Edward Hospital 801 S Washington Street Naperville, Illinois
Hours: Full time Will work 72 hours in a two week pay period. Nights 7:00pm-7:30am Rotate weekends/holidays

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

1. Required Education and/or Experience:

• Bachelor's Degree in Nursing or Bachelor's Degree in health care related field with case management experience
• A minimum of one year of previous case management and/or clinical specialist experience

1. Required License and/or Certification:

• Current State of Illinois Registered Nurse licensure
• Current CPR certification issued by either American Heart Association or American Red Cross

• Certification in case management
