Responsibilities:
- Provide case management services to patients, including assessment, planning, implementation, and evaluation of patient care plans.
- Collaborate with healthcare professionals to ensure coordinated and comprehensive care for patients.
- Conduct patient assessments and develop individualized care plans based on patient needs.
- Monitor and evaluate patient progress and make adjustments to care plans as necessary.
- Coordinate discharge planning and ensure smooth transitions for patients from hospital to home or other care facilities.
- Maintain accurate and up-to-date medical records and documentation.
- Advocate for patients' rights and ensure their needs are met throughout the healthcare process.
Requirements:
- Valid Registered Nurse (RN) license in [state].
