Rn - Case Manager

A Reputable hospital located in Eagleville, PA has an immediate need for a RN Case Manager.
Type: 13+ week Contract
Schedule: Days Monday – Friday 8a-4p or 9a-5p
Location: Eagleville, PA
Job Summary:
• Responsible for aftercare planning and aftercare Groups for all patients assigned to their case-load.
Essential Job Duties and Responsibilities:
• Meet in a timely manner with each patient to develop an aftercare plan.
• Conduct aftercare groups focusing on identifying aftercare needs, relapse and aftercare planning options.
• Provide family services when needed through family counseling and/or educational group modalities.
• Participate fully in Team Treatment meetings, Unit meetings, and Program meetings.
• Assist patients in maintaining contact with external systems where necessary in regard to Probation or Parole Officer, DPA, DHS, employer's funding resources, etc.
• Maintain all appropriate documentation and correspondence relative to each patient's individual medical record, according to Hospital, State and Federal Standards.
• Assists clients in maintaining contact with external systems when necessary including: Probation or Parole Officer, DPA, DHS, Employers, Funding Sources, and other case management as required. Completes Informed Consents for each contact person/agency as needed and documents properly.
• Consults with other staff members to insure quality patient care.
Requirements:
• Must have Active PA State RN License
• Bachelor of Science in Nursing (BSN) preferred but not required
• Must have 2 years Case Management experience
• Current BLS, ACLS and PALS certifications required for most units
• Experienced with discharge planning
• Need Acute Care Experience
