Job description

Job Summary The Transition of Care (TOC) RN Care Navigator under the direction of the Manager of Transitions of Care Department will provide support services in all functional areas of the TOC objectives and goals in an inpatient, outpatient and community settings. The TOC RN Navigator will serve as liaison between patients, healthcare professionals and community-based organizations and act as an expert resource person for all transitional and care management needs as it relates to the objectives and goals of Care Transition and population health care management.

Required Qualifications

  • 5 years of recent RN experience in a similar setting
  • 2 years of experience with work in Transitions of Care/care management/navigation
  • Experience in identifying and analyzing care/service needs for patients.
  • Graduate of an accredited RN program
  • Valid California Registered Nurse License without restriction.
  • Experience required in planning for short, immediate, and long-term goals and options.
  • Demonstrated knowledge with Windows-based software including Microsoft word, Excel, Outlook. Strong Knowledge of clinical based computer systems and software.
  • Knowledge of care/services provided in differing levels of care, i.e., ambulatory, and institutional settings, community-based resources, SNF, Home Health, Hospice, and Palliative Care.
  • Advanced patient care skills, working with a high volume and high expectations customer base. Strong ability to support patients with a wide variety of clinical and non-clinical needs.
  • CPR Certification from American Heart Association (AHA) or UCDH issued certification
  • BSN

Preferred Qualifications
  • Case Manager Certification
  • Experience in a role demonstrating use of superior communication skills including therapeutic listening, reflective listening and teach-back
  • Training in motivational Interview
  • Experience/familiarity with inpatient case management and inpatient patient flow

Department Description
The Transition of Care department aims to develop an effective and cost-efficient system of delivering and managing patient-centered, team-based quality care for those patients with chronic illnesses, multi hospitalizations, frequent emergency department visits, patients that present at risk of illness and or readmissions, and those patients with an acute, chronic and/or complex social influencers of health challenges. With high degree of autonomy, the TOC RN Navigator will work independently, but receives direction concerning the overall goals and objectives of the department and participates in actions to achieve these departmental goals. Utilizes specialized knowledge and skills within major areas of focus, such as patient) family, nurses and nursing practice, and the organization / system.

Physical Demands and Work Environment
  • Has mandatory reporting responsibilities for domestic violence, child abuse and elder/dependent abuse.
  • Ability to safely lift 25 pounds and assist in moving patients, and to stand on feet over extended periods.
